Abstract

The analysis of inaccuracies in software products of different manufacturers on the presentation of EEG coherence indicators was carried out, the essence of the problem was described in detail. The authors specifically marked the dependence of quantitative indicators of coherence depending on the regime conditions for their calculation. Variants of visual display of the results obtained depending on the objectives of the study are analyzed. An optimal variant of topographic mapping of coherence values and an algorithm for their clinical interpretation are proposed.

Abstract

Extraoperative invasive EEG-monitoring of bioelectric activity in cortical region and hippocampal complex may be performed in cases of discrepancy between clinical data, results of neurovisualization, and EEG-monitoring patterns when scheduling volume of surgical treatment in drug-resistant mesial epilepsy. A search for supplementary information about functional state of hippocampus may be directed to evaluation of its background bioelectric activity.

The aim of our study was to characterize amplitude and frequency parameters of hippocampal bioelectric activity in mesial temporal epilepsy.

Materials and methods. The study included analysis of amplitude and frequency parameters of background activity from the hippocampal complexes (21 tracks) performed during examination and surgical treatment of 17 patients (9 males and 8 females at the mean age of 31.5 years old) diagnosed with drug-resistant temporal mesial epilepsy. The two-year outcomes of selective amygdalae/ hippocampotomy in 7 patients were analyzed.

Results. The interictal background bioelectric activity of structurally altered hippocampal complexes is characterized by dominating slow-wave activity. By their capacity, the hippocampal tracks may be classified in two groups: 1) high-amplitude activity reflecting the process of epileptogenesis; 2) low-amplitude activity which suggests a “burnout” of structurally impaired hippocampus. Analysis of clinical outcomes following amigdalae/hippocampotomy has shown that the favorable outcomes were significantly more frequent upon destruction of epileptized hippocampus. Conclusions. The specified variants of EEG-patterns may be regarded as neurophysiological correlates of pathogenetic mechanisms underlying the drug-resistant mesial epilepsy. Hyppocampal complexes with high-amplitude background activity represent pathological generators which are lost upon “burnout” of their activity.

Abstract

Target. To investigate the effect of the EpiTapp® application on alpha rhythm synchronization in patients with genetic generalized epilepsy.

Materials and methods. The total sample size was 81 people, including: patients suffering from juvenile myoclonic epilepsy (main group, mean age — 2 7 [23; 38] years) —  30 people; healthy volunteers (control group, mean age —  39 [21; 56] years) —  51 people.

Results. The present study showed that wrist tapping using the EpiTapp® application statistically significantly reduced the degree of alpha synchronization in juvenile myoclonic epilepsy. At the same time, a decrease in the synchronization of the alpha rhythm is achieved in 66.7 % of cases.

Conclusion. Our results of changes in the bioelectrical activity of the brain after a session of wrist tapping indicate the possibility of using the EpiTapp® application as a non-drug rehabilitation method related to seizure relief programs in patients with juvenile myoclonic epilepsy.

Abstract

The relevance of the study is due to the complexity of the differential diagnosis of left anterior fascicular block (LAFB) with myocardial infarction and left ventricular hypertrophy based on a standard ECG.

Aim. Determination of the criteria for LAFB according to the data of three- dimensional vectorcardiography with an analysis of the rate of formation of the vector loop.

Material and research methods. The study included 85 patients who underwent a 12-lead ECG study. 2 groups of patients were selected. The main group of 56 patients with LAFB, mean age was 63.3±16.6 (M±δ) years, 34 men and 22 women. The control group included 29 healthy individuals, the mean age was 21.4±2.5 (M± δ) years, 12 men and 17 women. All patients underwent synchronous recording of 12 standard ECG leads in the IS «Unified Cardiologist of the Republic of Tatarstan» and reconstruction of a three-d imensional vectorcardiogram using the McFee-Parungao system with an analysis of the vector loop formation  rate using EasyECG Rest ATES Medica software (Russia).

Results and conclusions. It is shown that for LAFB the movement of the vector loop counterclockwise is typical in the frontal plane; the presence of 2 extrema, the second is directed upwards, to the right and back. On the graph of the rate of formation of a spatial vector loop, 2 peaks are recorded with a decrease in the rate in the final part of the QRS loop by 2–2.5 times relative to the first peak, which reflects a slowdown in myocardial excitation in the output section of the left ventricle.

Abstract

Integration of highly sensitive Doppler methods of assessment of coronary and cerebral blood circulation into process of functional and diagnostic audit of results of recovery treatment of patients with an arterial hypertension allows to estimate positive tendencies at normalization of processes regional perfusion. Use of the designated technologies showed reliable decrease in peak velocity in middle cerebra artery without increase in vascular resistance at the persons which passed a therapeutic course of antler reindeer breeding.

Peak velocity in an middle cerebra artery before the designated treatment was within 92–115 cm/ c (Me 105 [96,3–104,5]), after the therapeutic procedures was registered in a big way 78–98 cm / c (Me 90 [82,4–94,3]). The designated phenomenon indicated improvement of regional cerebral oxygenation and natural reduction of volume of brain perfusion. At the same time, Doppler parameters in coronary arteries at the patients who completed the analyzed course of recovery treatment were transformed minimum, without reliable distinctions that presumably.

Abstract

The article describes a clinical case of registration of longitudinal systolic deformation of the left ventricle using speckle-t racking technology during stress- echocardiography with transesophageal atrial pacing in a patient with coronary heart disease. A feature of the clinical case is the identification of changes in the longitudinal deformation of the left ventricular not only in the zone of visible local disturbances of myocardial contractility, but also in other zones, followed by the identification of hemodynamically significant stenoses of the coronary arteries, which supply the zones with defective longitudinal deformation of left ventricular, but without visible local disturbances of myocardial contractility. The presented case shows the prospects of using the evaluation of left ventricular deformation indicators during stress echocardiography with transesophageal atrial pacing.

Abstract

Speckle-tracking echocardiography is a promising modern technique for assessing structural and functional changes in the myocardium in coronavirus infection, which is important for long-term prognosis.

The aim of this study is to identify and evaluate myocardial effects in COVID-19 infected patients by echocardiography using specialized advanced imaging and a protocol to analyze the various components of myocardial deformity.

RESULTS AND METHODS An extended echocardiographic imaging protocol was performed in 57 patients with SARSCoV-2 infection, assessing longitudinal, radial, and circumferential LV deformity, including rotation, torsion, and torsion. In addition, LV deformity was analyzed in an age-matched control group of healthy individuals (n=20). The most predominant finding was a reduction in longitudinal deformity, predominantly in more than one LV basal segment (n=39/57 patients, 69 %). This pattern resembled the “reverse tako-tsubo” morphology, which is not typical of other viral myocarditis. Additional findings included a biphasic pattern with maximal post-systolic or negative regional radial deformity, predominantly basal (n=21/57 patients, 37 %); absence or dispersion of LV basal rotation (n=28/57 patients, 50 %); reduced or positive regional circumferential deformity in more than one segment (n = 31/57 patients, 50 %).

Conclusion. Myocardial involvement caused by SARS-CoV-2 can be characterized by specific patterns of LV deformity, even in patients with mild/moderate symptoms. Detection of myocardial involvement by speckle tracking echocardiography is possible in the acute stage of COVID-19, which may improve early detection of myocardial injury, risk of stratification, and potentially cardioprotective treatment.

Abstract

Development of medical imaging techniques (including medical thermography) provided an opportunity to mine data that can be explored for the development of clinical decision support systems.

Aim. Find out quantitative thermography criteria for acute rhinosinusitis and implement these criteria into automated analysis protocols to create a diagnostic complex suitable for use by professionals of any medical speciality.

Material, methods. After necessary adaptation, facial thermography by mean of thermography camera ТВС300-меd LLC “СТК СИЛАР” Russia, (384x288 pixel, 30 mK) was performed in 100 healthy volunteers and 305 patients with suspected acute rhinosinusitis (in 173 of them diagnosis was supported). Resulted thermograms were processed in “cloud service” TVision (LLC “Dignosys”, Russia). Point and regions of interest (ROI), namely, point in the middle of the line, connected inner edge of eyebrows (Тср) and ROI, covered the projections of maxillary sinuses were marked automatically. Difference between temperature in Тср point and mean temperature in each ROI (ΔТr/ΔТl), as well as mean temperature between ROI were calculated (ΔTпаз).

Result. The best results ( sensitivity 82%, specificity 74%, accuracy 78%) were received when complex of ΔТr/ΔТl ≤0 and module |ΔTпаз| ≥0,5°C (both or at least one criterion) was applied. Based on these data, automatic algorithm of thermography acute rhinosinusitis diagnostics was created to use in primary medical care routine as clinical decision support systems.
