Abstract

The changed realities of modern health care, including the reform, the abolition of internship, the development of the outpatient and polyclinic direction of the development of domestic medicine dictate the need to develop and introduce new forms of doctor education and professional training throughout the entire period of activity. The authors have consistently created a scientific and Educational Project ‘Outpatient reception’ of the Regional Public Organization for Promoting the Development of Prehospital Medicine ‘Outpatient doctor’ and a Scientific and Practical Centre for Training and Continuous Professional Development of Primary Care Specialists on the basis of the City Clinical Hospital n.a. S. I. Spasokukotsky with the participation of Moscow State University of Medicine and Dentistry n.a. A. I. Evdokimov. The article gives an idea of these projects, describes individual areas of activity.

Results. For several years of active work, the proposed approaches have proven their viability and relevance for the medical audience, the number of listeners and active participants in educational events is growing, new textbooks and training materials are being published aimed at improving the provision of medical care in the Russian Federation.

Abstract

Within the framework of the Federal Project ‘Creation of a unified digital contour in health care based on the unified state information system of health care’ of the National Project ‘Healthcare’, employees of the Moscow State Medical and Dental University n.a. A. I. Evdokimov (Department of Therapy, Clinical Pharmacology and Emergency Medicine) with the assistance of RPO ‘Outpatient Doctor’ in 2020, a system for supporting medical decisions was created. The program ‘Hippocrates’ works in the testing mode, but now it is fulfilling the tasks assigned to it to increase the early detection of chronic diseases, and it also plays an important role in teaching doctors to take action based on clinical guidelines.

Abstract

The article is a variant of an interdisciplinary educational dialogue between an infectious disease doctor and a therapist and aims to increase the awareness of therapists in terms of diagnosis and treatment of therapeutic and infectious diseases occurring with the presence of hepatolienal syndrome. The most common pathology is briefly presented, algorithms for differential diagnostic search are given.

Abstract

Jaundice is a syndrome that occurs in the practice of doctors of all specialties, accompanies many diseases and pathological conditions. The onset of jaundice, regardless of the cause, is a formidable sign that requires immediate medical attention. Often, hyperbilirubinemia indicates the presence of a serious illness that threatens a person’s health or life. Therefore, the determination of the presence of jaundice, the assessment of its accompanying symptoms contributes to the correct diagnosis of the underlying disease, accompanied by this syndrome, the appointment of adequate therapy, which can improve the patient’s quality of life, and sometimes save life.

Abstract

Difficulties in treating patients with rheumatological diseases are due to the complexity and versatility of pathogenesis, frequent comorbidity and advanced age of the majority of such patients. The use of non-pharmacological methods of treatment, including physical methods, enhances the analgesic and anti-inflammatory effect, improves tissue trophism and promotes the activation of reparative processes, slows down the progression of the disease and generally improves the quality of life of patients. In the complex treatment and rehabilitation of rheumatological patients, the use of magnetotherapy seems to be a method that is well substantiated by the data of numerous fundamental and modern clinical studies.

Abstract

Pain syndrome accompanies the vast majority of diseases; therefore, the issues of adequate pain relief remain topical not only for urgent conditions, but also for everyday medical practice. Modern and changed in recent years approaches to the appointment of anesthetic therapy should take into account not only the pathogenetic mechanisms of the development of pain syndrome in a particular patient, but also the need to use drug combinations. This allows for the potentiation of the analgesic effect, reduction of effective dosages of individual drugs and minimization of side effects. In case of severe pain syndrome in the presence of signs of impaired nociception, neuropathic and muscle-fascial pain syndromes, the use of non-steroidal anti-inflammatory drugs in conjunction with group B vitamins is justified. The article considers the pathogenetic details of such a combination therapy, reveals the mechanisms of the cooperative action of the proposed combination of drugs.

Abstract

Objective. Increasing the detection rate of oncological diseases in the early stages using questionnaires.

Materials and methods. The study involved 25,467 patients who came to an appointment with a general practitioner / general practitioner for any request. During the study, primary care physicians, regardless of the purpose of the visit, purposefully asked questions regarding the risks of malignant neoplasms.

Results. The detection rate of oncological diseases was 60.4% in the first half of 2018 against 56.3% in the same period of 2017. In accordance with specific localizations, the following results were obtained: detection of stomach cancer increased by 3%, colon cancer by 2%, malignant neoplasms of the trachea, bronchi and lungs increased by 6%, and the mammary gland, cervix and prostate increased by 1%, 8%, and 2%, respectively.

Conclusions. The presence of a clear stereotype of oncological alertness in the primary care physician may contribute to a decrease in the number of advanced stages of tumors due to the initiation of specialized treatment at earlier stages. In such a situation, the main task of the therapist and general practitioner is to identify suspicious symptoms of malignant neoplasms using an algorithmized approach and refer the patient to a specialized institution.

Abstract

The pandemic of the new coronavirus infection COVID‑19, according to recent studies, has a significant effect on vascular platelet hemostasis in its pathogenesis. The article examines the tactics of managing patients with COVID‑19 from the standpoint of assessing hemostasis disorders, the risks of thrombus formation and the correction of detected disorders.

Abstract

According to the who, the share of acute respiratory viral infections (амма? and influenza) accounts for about 90–95% of all infectious diseases; in russia, influenza and arvi take up to 40% of the total duration of official disability, which determines the significance of this pathology. At the same time, the primary contact of the overwhelming majority of patients with suspected arvi and influenza occurs with a therapist or general practitioner of polyclinics. The proposed consensus of experts is intended to systematize the known approaches to the diagnosis, treatment and secondary prevention of influenza, acute respiratory viral infections and community-acquired pneumonia for their use at outpatient clinics. The schemes of using interferon therapy for arvi and influenza are considered in detail.
