Abstract
The review focuses on 15-year studies of bevacizumab in metastatic colorectal cancer. It is shown that the drug is a universal chemotherapeutic partner and can be prescribed depending on the clinical situation and the goals of treatment with both low-intensity and standard and high-intensity modes of chemotherapy, regardless of the presence or absence of mutations in the RAS and BRAF genes. The proven possibility of using bevacizumab in maintenance therapy (preferably in combination with fluoropyrimidine) allows systemic treatment of patients with unresectable metastases within the modern strategy of de-intensification without reducing survival rates. Continued use of bevacizumab after the progression of the disease with a change in the chemotherapy regime increases the life expectancy of mCRC patients. The drug can be prescribed for tumors of both right-sided and left-sided localization, and ongoing studies to determine the relationship of molecular CRR subtypes and the effectiveness of targeted therapy will help improve the results achieved to date.

Abstract
According to published randomized studies of second-line therapy in patients diagnosed with gastric cancer, an increase in the median survival with chemotherapeutic agents (docetaxel or irinotecan) is approximately 1.5 months compared with symptomatic therapy. There are data from comparative studies evaluating the effectiveness of other targeted drugs: the use of everolimus as second-line drugs did not reveal a significant increase in OS; studies of anti-EGFR therapy (the addition of panitumumab or cetuximab to chemotherapy) as first-line therapy also did not show a significant increase in OS or PFS, respectively. The increase in OS in patients treated with a combination of ramucirumab and paclitaxel, in comparison with the placebo group and paclitaxel, can be considered clinically significant for this patient population and will allow treating this therapeutic regimen as a new standard of second-line treatment in patients with this nosology.

Abstract
The article gives a review of the historical reference and presents the innovative approaches to intraoperative radiation therapy (IORT) procedures as effective method of oncology treatment. The authors summarize big experience to use this method in the different cancer diseases, estimate the influence on locoregional control, disease-free survival and overall survival, quality of life, toxicity, postoperative mortality. Also authors describe the features of dose planning, perspectives of IORT: stereotactic navigation, intraoperative imaging, and integration of laparoscopic and pre-robotic techniques with this method.

Abstract
According to achievements in clinical oncology the percentage of older adults undergoing cancer therapy has increased. Oncologists usually estimate the age only to focus on comorbidity, while we should use Comprehensive geriatric assessment (CGA). Special geriatric team should participate in implementation and interpretation of this process. Mini Nutritional Assessment (MNA) is offered for nutrition assessment for geriatric oncology patients. Nutrition interventions in geriatric oncology should have anabolic strategy. Older patients with cancer should consume a minimum of 1.2-2.0 g of high quality protein (kg per day). Exercise must be integral part of the supportive care management of older adults with cancer.

Abstract
The article analyzes the results of treatment of patients with a diagnosis of primary operable breast cancer who were on treatment at the Samara Regional Clinical Oncology Centre. It was proved that the performance of radical resections does not lead to worsening of the immediate and long-term results of treatment of patients diagnosed with breast cancer in comparison with the group undergoing radical mastectomies. Modern methods of radical resections do not reduce the social activity of patients.

Abstract
Inthrathecal synthesis (ITS) during acute lymphoblastic leukemia (ALL) and neuroleukemia (NL) in children is very important. Therefore, we try catch and assess degree of ITS in children with ALL and NL at time of investigation, condition of blood-brain barrier (BBB). Our method s investigation of BBB and cerebrospinal fluid (CSF) are as radioactive nuclide and neurospecific proteins researches. Research’s parameters - RP (protein, albumin, globulines, aminonitrogen, neuroactive aminoacids - glutamic acid, glutamine in CSF and serum [S] of blood). During ALL (status CNS-1): acute period - AP, remission - Rm, recurrence and NL: preclinical and clinical variants NL (CNS-3), initial NL in children (CNS-6). Concentration ’s gradients CSF/S for albumin, aminonitrogen, glutamic acid, glutamine in ALL, NL were determine. ITS have place in all periods ALL from AP indicated occult NL (ONL) and in deep immunodepression periods in CNS during high dose BFM-90 Protocol (Germany) therapy, increasing accordingly remission from year to years (due chemical meningosis)
