Disoder of breathing during sleep in clinical practice of therapist

Abstract

The aim of the study was to evaluate clinical and anthropometric parameters in outpatients of ^є^рє^Ь рюАіє, depending on the presence or absence of disorders of breathing during sleep according to Computer риIse oximetry. Methods. The study included 94 outpatients aged 30 to 70 years. Exclusion criteria: obstructive puImonary diseases, acute violation of cerebral circulation, chronic heart failure III and IV functional class, severe pathoIogy of ENT-organs, anemia. Patients were interviewed on the presence of clinical symptoms of disorders breathing during sleep, carried out laboratory examination, computed pulse oximetry (CPO). Results. CPO was held on 30 of 94 (32 %) outpatients. Chronic hypoxemia according to the manual CPO was identified in 20 (66.7 %) patients. Medium and high risk of nocturnal hypoxemia was registered in 16 (53.3 %) patients. Patients with hypoxemia were older, BMI and waist circumference were higher than in patients without hypoxemia. Factors which can have an effect with nocturnal hypoxemia were found to be high BMI (р = 0.04; r = 0.38), excess waist circumference (р = 0.002; r = 0.55) and neck circumference in male (р = 0.009, r = 0.46), high level of pulse BP (р = 0.003, r = 0.38) and systolic BP (р = 0.031, r = 0.38), the presence of coronary artery disease (CAD, р = 0.036; r = 0.40) and the metabolic syndrome (р = 0.003; r = 0.38), the erythrocytosis (р = 0.001; r = 0.62), high cholesterol (р = 0.026; r = 0.42) and NTрю-BNP level (р < 0.001; r = 0.67). Conclusion. The combination of overweight, metabolic disorders, CAD should cause suspicion among internists and general practitioners in relation to the presence of nocturnal hypoxemia with the purpose of further examination to identify disorders of breathing during sleep.
