Карцинома Меркеля – редкая злокачественная опухоль кожи (на примере двух клинических наблюдений)

Аннотация
В данной статье представлен обзор литературы по распространенности карциномы Меркеля, особенностях ее клинической, дерматоскопической, патоморфологической диагностики и терапии. В свете представленных данных описаны два собственных клинических наблюдения карциномы Меркеля: даны клинико-дерматоскопическое описание, патоморфологическая и иммуногистохимическая картина, проведенное лечение и его отдаленные результаты. В представленных клинических наблюдениях карцинома Меркеля была выявлена у двух женщин 70 и 83 лет на I стадии, что определило благоприятный прогноз. Клинически опухоль была представлена в виде плотного неизъязвленного узла или узелка розово-красного цвета, все новообразования локализовались на голове. Дерматоскопическая картина была различна, в одном случае была представлена глобулами и точками красного цвета на фоне молочно-розовых и белых областей, отдельными линейными и извитыми сосудами, присутствовали белые структуры (по типу хризалидов), а в другом случае – бесструктурные области розового цвета с древовидными сосудами и белыми структурами в виде хризалидов. Гистологически преобладал мелкокруглоклеточный тип строения. При ИГХ-исследовании в обоих случаях выявлена выраженная экспрессия цитокератина 20 и хромогранина А, индекс Ki‑67 был выше 70 %.
