Современные представления о кластерной головной боли. Обзор литературы

Аннотация
В статье представлен обзор литературы где, начиная с публикации Ekbom K. (1947 год), описана история выделения кластерной головной боли в отдельную форму первичной головой боли, представлены современные критерии диагностики, описаны патогенез, клинические особенности заболевания. Показано, что в связи с редкостью этого вида головной боли и малой осведомленностью врачей, задержка в диагностике в среднем составляет 6–7 лет. Пациентам назначают массу ненужных обследований и консультаций специалистов: окулистов, ЛОР-врачей, нейрохирургов, психиатров и др. Излюбленными неверными диагнозами являются мигрень и невралгия тройничного нерва. Приведены доказательства ведущей роли гипоталамуса в патогенезе кластерной головной боли. Обсуждаются принципы терапии: для купирования острых приступов используют, как и при мигрени, триптаны, лидером профилактической терапии является блокатор кальциевых каналов верапамил . При хронических формах и резистентных к фармакологическому лечению приступах используют методы нейростимуляции.
