Abstract
The rewiew is dedicated to the actual problem of treatment of pain syndrome in elderly patients with arterial hypertension and osteoarthritis. The article describes the mechanism of action, effectiveness, specific advantages and disadvantages of non-steroidal anti-inflammatory drugs. Clinical data on semiselective cyclooxygenase-2 (COX-2) inhibitor meloxicam are discussed. The cardiovascular and gastrointenstunal safety of meloxicam has been shown in elderly multimorbidity paitients.

Abstract
Recently infection safety in healthcare has become emerging problem. Norms and guidelines, first of all, SANPIN, describe the basic principles of epidemiological supervision in details. Despite this, official recommendations are not completely clear to clinicians and they often follow them just in formal way. Simultaneously, lack of HAI statistic and epidemiology doesn ’t allow to consider efficacy of implemented programs, especially in outpatient facilities.What is seriously affect the HAI incidence in policlinics, which measures should medical staff take into consideration as crucial ones? This article is concentrated on these questions. We analyzed the international practice of HAI prevention in outpatient care in countries with high level of HAI registration.

Abstract
Arterial hypertension is the leading risk factor for the development of cardiovascular disease. The violation of the circadian blood pressure profile is associated with an increased risk of cardiovascular complications. Early detection and treatment of hypertension has a beneficial prognostic effect. The article studies the influence of long-term stress on the daily profile of blood pressure in women, mothers of children with severe diseases. The data obtained allow to characterize the features of blood pressure in persons exposed to long-term psychoemotional stress.

Abstract
Gastroesophageal reiflux disease is a widespread pathology. The management of patients with GERD is not limited to gastroenterologists, but also to general practitioners and therapists. For effective treatment, it is important to know the drug and non-drug methods of treatment. Non-drug treatment includes modification of risk factors that trigger an exacerbation: obesity, smoking, body torso. The diet is an important method of treatment. Treatment includes the prescription of inhibitors of the proton pump, antacids, prokinetic. If Helicobacter pylori is detected, eradication therapy should be carried out. One of the frequent reasons for refractory to standard therapy is gastroduodenal reflux. The addition of ursodeoxycholic acid to treatment increases the effectiveness of therapy.

Abstract
Cardiovascular complications are often observed in the postoperative period. Surgery is a stress for the body. In addition to physiological stress, occurred by surgery, the patient s psychological state can influence on the postoperative period. The study revealed changes in the 24-hour blood pressure monitoring data and Holter monitoring data in patients with different stress resistance.

Abstract
Hypertension is classified as either salt-resistant or salt-sensitive. The study investigated the relationship between salt sensitivity and polymorphisms of AGT genes: 704 T>C, AGT: 521 C>T, AGTR 1: 1166 A>C, AGTR2: 1675 G>A, ADD 1: 1378 G>T, CYP11B2: -344 C>T, GNB3: 825 C>T, NOS3:-786 T>C, NOS3:894 G>Tamong the European population of Moscow city. A bioimpedance study confirmed that water retention is one of the leading mechanisms for the development of hypertension in salt-sensitive patients. In patients with water retention with the TT genotype and allele T of CYP11B 2 gene and the genotype of the TT of the NOS 3 gene an increased blood pressure was observed.The association of salt-sensitive hypertension with allele T of polymorphism of AGTgene (T704C) and allele of C polymorphism of gene GNB 3 (C 825ty was determined. There was revealed an association of high salt-taste threshold with the heterozygous genotype G/T of the ADD 1-а G1378T gene (Gly460Trp). These results allow the use of genetic markers in early personalized prevention of hypertension.

Abstract
The main cause of death in Moscow is cardiovascular diseases. Prevention and treatment of hypertension is one of the most important tools for reducing cardiovascular mortality. Reducing the incidence, increasing the detection and control of hypertension can be achieved only by combining organizational and legal measures with improving interaction in the doctor-patient system, i.e. with increased complience.
