Cytokine profile and predictors of the efficacy of genetically engineered biological therapy for psoriasis and psoriatic arthritis: a literature review

Abstract

Psoriasis and psoriatic arthritis (PsA) are recognized as heterogeneous immune-mediated inflammatory disorders with a pronounced systemic component, whose development is driven by complex interactions between innate and adaptive immunity. The IL-23/Th17/IL-17 axis plays a central role in the pathogenesis of psoriatic disease; however, increasing attention is being directed toward non-axis mediators, including IL-6, IL-8, IL-22, IL-33, and IL-35, which reflect tissue specificity and immunological heterogeneity. The introduction of TNF-α and IL-17A inhibitors has significantly improved both cutaneous and articular outcomes; nevertheless, variability in clinical response and secondary loss of efficacy remain substantial challenges. Evidence from clinical and immunological studies demonstrates heterogeneity in the dynamics of circulating cytokines during biologic therapy and underscores the need for reliable predictive biomarkers of treatment response. A comprehensive assessment of cytokine profiles, together with accessible hematological markers of systemic inflammation- particularly the neutrophil-to-lymphocyte ratio (NLR) – may represent a promising approach for disease monitoring and the advancement of personalized therapeutic strategies.
