Основные современные методы лечения генитоуринарного менопаузального синдрома

Симптомы генитоуринарного менопаузального синдрома (ГУМС) появляются одновременно с вазомоторными симптомами при наступлении менопаузы и даже в перименопаузе: болезненность, зуд и сухость во влагалище, диспареуния и т.д. Это связано с развитием атрофических изменений в урогенитальном тракте, что резко снижает качество жизни женщин и является фактором риска развития рецидивирующих инфекций мочеполовой системы. Более 65 % женщин в постменопаузе без системной менопаузальной гормональной терапии (МГТ) страдают проявлениями урогенитальной атрофии. В данной статье рассматриваются современные способы лечения ГУМС, описываются различные методы лечения, их эффективность, длительность применения. В качестве первой линии терапии рекомендуются негормональные смазочные гели. При средней и тяжелой степени вульвовагинальной атрофии и при отсутствии противопоказаний применяют локальное интравагинальное введение эстриола. Описаны и новые перспективные методы лечения ГУМС: оспемифен – пероральный активный селективный модулятор рецепторов эстрогенов (SERM), лазерная терапия, дегидроэпиандростерон.

Выводы. Генитоуринарный менопаузальный синдром – недооцененное распространенное заболевание, но частота применения локальных эстрогенов все еще низкая. Лечение следует продолжать длительно для предупреждения рецидивов симптомов ГУМС. Вагинальные увлажнители могут использоваться как в монотерапии, так и в сочетании с эстриолом.
