Хроническая боль у пациентов пожилого возраста

Аннотация
Цель обзора. Статья посвящена хроническому болевому синдрому (ХБС) у пациентов пожилого возраста и возможностям различных терапевтических стратегий у этой категории больных.
Основные положения. Хроническая боль и биологический процесс старения имеют сходные патофизиологические (клеточные и молекулярные) механизмы развития. Однако хроническая боль не является неизбежной составляющей процесса старения, однако гораздо чаще представлена у лиц старшего возраста, диагностика и терапия которых сопряжены с атипичными клиническими проявлениями боли у пожилых пациентов и необходимостью более внимательного, взвешенного подхода при оценке фармакокинетических и фармакодинамических изменений, связанных с процессом старения. Для обеспечения достаточного обезболивания применяются мультидисциплинарный подход и соответствующие методы терапии.
Заключение. Итоговый результат лечения ХБС (снижение интенсивности боли, восстановление функциональной активности, автономности и др.) зависит от полиморбидности, гериатрического статуса и когнитивных возможностей пациента, поэтому необходимо учитывать все имеющиеся факторы для адекватной и полноценной терапии боли.
