Деэскалация системной терапии раннего и местно-распространенного тройного негативного рака молочной железы

Аннотация
Неоадъювантная химиотерапия (НАХТ) раннего и местно-распространенного тройного негативного рака молочной железы (ТНРМЖ) является обязательным звеном в составе радикального лечения. Достижение полного патоморфологического регресса (пПР) определяет прогноз заболевания, позволяет провести органосохраняющее лечение, а также снижает риск рецидива и улучшает общую выживаемость. В последнее время наблюдается увеличение количества исследований, направленных на интенсификацию НАХТ, что объясняется известной агрессивностью данного подтипа рака молочной железы. Однако, увеличение частоты и тяжести нежелательных явлений, часто нивелирующих улучшение эффекта лечения, привели к появлению исследований, направленных на снижение химиотерапевтической нагрузки НАХТ ТНРМЖ без потери его эффективности. Изучаемые схемы химиотерапии могут стать новыми стандартами неоадъювантного этапа комплексного лечения резектабельных форм ТНРМЖ. В этом обзоре рассмотрен текущий стандарт системного лечения раннего и местно-распространенного ТНРМЖ, а также возможные варианты деэскалации лекарственной терапии.
