Эффективность терапии больных псориазом с тревожностью и депрессией ингибитором интерлейкина‑23 гуселькумабом

Псориаз – хроническое иммуноопосредованное системное заболевание, которое тесно связано с коморбидной депрессией и тревожными расстройствами. Сегодня новое направление терапии представлено биологической терапией. Одним из эффективных и безопасных препаратов является ингибитор ИЛ‑23 гуселькумаб.

Цель исследования. Изучение эффективности терапии ингибитором интерлейкина‑23 (гуселькумабом) и динамики показателей симптомов тревожности и депрессии у пациентов со среднетяжелым и тяжелым псориазом.

Материал и методы. Проведено ретроспективное исследование 25 пациентов с диагнозом «распространенный вульгарный псориаз». Исходные показатели оценивали по индексам PASI, BSA, sPGA, DLQI, ВАШ (шкала зуда). Показатели тревожности и депрессии оценивали по шкалам HADS и GAD‑7. Все пациенты получали лечение гуселькумабом по стандартной схеме. Оценку эффективности терапии осуществляли по динамике показателей PASI, BSA, sPGA, DLQI, ВАШ, HADS, GAD‑7 через 12 недель терапии ингибитором интерлейкина‑23 (гуселькумабом).

Результаты. Биологическая терапия гуселькумабом привела к значительному снижению всех показателей через 12 недель для каждого пациента (р < 0,0001). Только улучшение PASI значимо коррелировало с более низкими показателями депрессии и тревоги (p < 0,0050), тогда как снижение DLQI не коррелировало (p > 0,9550). Заключение: биологическая терапия ингибитором интерлейкина‑23 гуселькумабом высокоэффективна как для снижения тяжести заболевания, так и облегчения симптомов депрессии и тревоги при псориазе.
