Эмоционально-личностные особенности и качество жизни у женщин с синдромом раздраженного кишечника

Цель исследования. Изучить эмоционально-личностные особенности и качество жизни у женщин с СРК, в зависимости от возраста и учитывая варианты СРК. Материал и методы. Обследованы 245 больных с СРК, из которых женщин - 783, мужчин - 62, средний возраст 45,31 ± 6,9 года (79-87), клинические формы (СРК по варианту диареи (СРК-Д) - 88, СРК по варианту запоров (СРК-З) - 48, СРК по смешанному варианту (СРК-СМ)- 709. Анализ был проведен в двух группах женщин с СРК: до 45 лет (n = 90) и от 45 лет и выше (n = 93). Диагностические критерии СРК согласно Рим III (2006). Показатели депрессии по шкале БЭКа, тревоги по Спилбергера-Ханина, алекситимии по опроснику TAS и качество жизни по опроснику MOS SF-36. Результаты. СРК у женщин встречается значительно чаще, чем у мужчин, соотношение женщин и мужчин составляет 3:7, независимо от возраста. СРК по запорному варианту встречается исключительно у женщин в обеих возрастных группах. Показатели депрессии и личностной тревоги значимо превышают аналогичные показатели у мужчин (р < 0,05) преимущественно за счет женщин возрастной группы от 45 лет и выше. Также у женщин от 45 лет и выше значимо возрастают депрессия, личностная тревога и алекситимия по сравнению с женщинами до 45 лет (р < 0,05). С возрастом показатель алекситимии значимо повышается как у мужчин, так и у женщин с СРК (р < 0,05). Физический компонент здоровья у женщин значимо снижен по сравнению с мужчинами, независимо от возраста (р < 0,05). В группе женщин до 45 лет максимальные значения депрессии, личностной и актуальной тревоги наблюдаются при запорном варианте СРК, в группе женщин от 45 лет и выше при СРК-Д. Выводы. СРК преобладает у женщин по сравнению с мужчинами, независимо от возраста. Изменения психовегетативного статуса, в частности, депрессия и личностная тревога также более выражены у женщин, особенно у женщин в возрасте от 45 лет и старше. Физический компонент здоровья значительно более снижен у женщин по сравнению с мужчинами, независимо от возраста.
