Цель работы. Определение взаимосвязи развития признаков депрессии, частоты выявления пародонтопатогенных бактерий и спектра цитокинов, играющих вероятную роль в патогенезе гингивита у студентов медицинского вуза в условиях академического стресса. Материалы и методы исследования. Проведено психологическое тестирование, клиническое стоматологическое и лабораторное обследования 71 студента медицинского университета (41 женщина, 30 мужчин) в возрасте от 19 до 22 лет. Выявление депрессии проводили с помощью шкалы А. Бека, реактивной и личностной тревожности - Ч. Спилбергера - Ю.Л. Ханина. Для оценки состояния пародонта использовали традиционные гигиенические и пародонтальные индексы (OHI-s, РНР, РМА). Выявление маркерной ДНК пародонтопатогенных бактерий проводили с помощью полимеразной цепной реакции с использованием тест-набора «Мультидент-5» производства НПФ «ГенЛаб» (Россия). Содержание цитокинов в слюне определяли с помощью твердофазного иммуноферментного анализа. Для детекции ИЛ-Iβ, ИЛ-4, ИЛ-10, ИЛ-12, ИФНγ, ФНОα использовали наборы НПО «Вектор Бест» (Россия), для цитокина ТФРβ - фирмы Biosource (Канада). Результаты. Установлено, что развитие депрессии и гингивита сопровождают изменения цитокинового статуса, основное патогенетическое значение имеют ИЛ-β и ТФРβ, а вспомогательное - ИЛ-12 и ФНОα. При этом цитокины ИЛ-1β и ФНОα связаны с тяжестью течения гингивита. Установлена корреляция для цитокинов ИЛ-1ß, ФНОα и ТФРβ с пародонтопатогенными видами первого порядка A acfinomycefemcomifans и T. forsythia.
Purpose. Determination of the relationship between the development of symptoms of depression, frequency of detection of pathogenic bacteria of periodontal and the spectrum of cytokines that play a probable role in the pathogenesis of gingivitis in students of a medical university in terms of academic stress. Maferials and mefhods. The have been conducted psychological testing, clinical dental and laboratory survey in 71 students of a medical university (41 women, 30 men) aged from 19 to 22 years. For the assessment of periodontal we used traditional hygiene and periodontal index (OHI-s, PHP, PMA). Identifying marker DNA of pathogenic bacteria of periodontal was performed using polymerase chain reaction with the test set ‘Multident-5’ produced by Genlab Co. (Russia). Cytokines levels in saliva were determined using enzyme linked immunosorbant assay. For the detection of IL-1β, IL-4, IL-10, IL-12, IFN-γ, TNF-α we used sets produced by Vector-Best Co. (Russia), and for the detection of cytokine TGF-β by Biosource Co. (Canada). Resulfs. It is established that the development of depression and gingivitis is accompanied by changes in cytokine status, the main pathogenetic importance of IL-1β, TGF-β and the sub importance of IL-12 and TNF-α. In the process cytokines IL-1β and TNF-α are associated with severity of gingivitis. A correlation for cytokines IL-1β, TNF-α and TGF-β with parodonto-pathogenic types of the first order A. acfinomycefemcomifans and T. forsyfhia was determined.
