Улучшение результатов лечения больных раком большого дуоденального сосочка. Материалы и методы. Проанализированы 253 больных раком Фатерова сосочка. У 6 пациентов опухоль была нейроэндокринной, в связи с чем они были исключены из исследования. Хирургическое лечение проведено 206 (81 %) пациентам, комбинированное - 47 (19 %). 142 пациентам выполнен пересмотр биопсийного материала и проведено иммуногистохимическое (ИГХ) исследование, по результатам которого в 50 % случаев опухоль имела интестинальный тип (ИТ), в 45,8 % - панкреато-билиарный тип (ПБТ) и в 4,2 % опухоль была нейроэндокринной. Проведены однофакторный и многофакторный анализы выживаемости в зависимости от демографических, патоморфологических и клинических характеристик. Результаты. Независимую роль показали семь факторов: патоморфологический тип опухоли, размер опухоли, метастазы в регионарных лимфатических узлах, продолжительность операции, наличие осложнений со стороны культи поджелудочной железы, вид соустья с культей поджелудочной железой (с желудком или кишкой), метод лечения (хирургическое, комбинированное). Причем отдаленная выживаемость достоверно ухудшается при увеличении количества негативных независимых факторов прогноза. При ПБТ рака БДС комбинированное лечение с адъювантной химиотерапией обусловливает показатели общей выживаемости (трехлетняя - 65 ± 15 %, медиана 37 месяцев), достоверно превышающие таковые (49 ± 8 % и 35 месяцев) после хирургического лечения, в то время как при ИТ различий в выживаемости не было. Выводы. ПБТ рака БДС характеризуется худшей выживаемостью по сравнению с ИТ. Адъювантная химиотерапия у больных ПБТ рака БДС улучшает выживаемость.</p></abstract><trans-abstract xml:lang="en"><p>Goal. To improve results of surgical treatment of patients with Papilla Vateri cancer. Materials and methods. 253 patients with Papilla Vateri cancer were identified between years 1976 and 2017 at the Surgical Department of Pancreas and Liver Cancer of National Medical Research Centre of Oncology n.a. N. N. Blokhin in Moscow, Russia. Results. Multivariate analysis with Cox regression model revealed that 7 factors were independently associated to a longer overall survival, which included intestinal pathomorphological type of the tumor, tumor size ≤ 2 cm, no lymph node involvement, time of operation ≤ 270 min., no pancreatic stump complications, pancreaticogastrostomy vs. pancreaticojejunostomy, method of treatment (combined treatment vs. surgery alone). The increase of amount of prognostic factors leads to a shorter overall survival. The combined treatment of patients with pancreato-biliary type of Papilla Vateri cancer results in better survival comparing to surgery alone (3-year survival 65 ± 15 % vs. 49 ± 8 %, median 37 vs. 35 months respectively; p = 0.006), while patients with intestinal type of cancer showed no difference in these groups. Conclusions. Pancreato-biliary type of cancer is associated with poorer survival comparing to intestinal type.
