В статье представлены два клинических случая использования экстракорпоральной мембранной оксигенации в комплексном лечении вторичного острого респираторного дистресс-синдрома при неэффективности и невозможности проведения традиционной интенсивной терапии. Первый случай применения вено-венозной экстракорпоральной мембранной оксигенации у пациента с тяжелой пневмонией, осложненной развитием острого респираторного дистреес-синдрома и двухстороннего напряженного пневмоторакса. Второй случай применения экстракорпоральной мембранной оксигенации у пациентки с осложненным течением послеоперационного периода кесарева сечения в виде развития синдрома массивной гемотрансфузии с повреждением легких и двухсторонним напряженным пневмотораксом. экстракорпоральная мембранная оксигенация является мерой временного эффективного замещения легочной функции при невозможности и неэффективности проведения искусственной вентиляции легких.
The article presents two clinical cases of using extracorporeal membrane oxygenation in the complex treatment of secondary acute respiratory distress syndrome at the ineffectiveness and inability of the traditional intensive care. The first case of veno-venous extracorporeal membrane oxygenation in a patient with severe pneumonia complicated by the development of acute respiratory distress syndrome and bilateral intense pneumothorax. The second case of extracorporeal membrane oxygenation in a patient with complicated postoperative cesarean section in the form of development of a syndrome of massive blood transfusion with damage to the lungs and bilateral intense pneumothorax, extracorporeal membrane oxygenation is a measure of temporary effective replacement of pulmonary function in the impossibility and inefficiency of artificial lung ventilation.
