Провести сравнительную оценку распространенности и интенсивности кариеса зубов, кариесрезистентности эмали и pH смешанной слюны у детей 6–7 и 12 лет, воспитывающихся в семьях и учреждениях интернатного типа. Материал и методы. Проанализированы исходные данные проспективного наблюдательного исследования, включившего 484 ребенка 6–7 и 12 лет. Основную группу исследования составили 287 детей-сирот и детей, оставшихся без попечения родителей, проживающих в трех учреждениях интернатного типа Московской и Ярославской областей, а группу сравнения – 197 учащихся московской школы № 1206, воспитывающихся в семьях. У детей в возрасте 6–7 лет интенсивность кариеса оценивали по комбинированному индексу КПУ+кп, у детей 12 лет – по индексу КПУ. Распространенность рассчитывали как долю детей, имевших хотя бы один кариозный, пломбированный или удаленный вследствие кариеса зуб. Наряду с распространенностью кариеса оценивали соотношение кариозных, пломбированных и удаленных зубов, pH смешанной слюны и кариесрезистентность эмали по ТЭР-тесту. Результаты. У детей основной группы в возрасте 6–7 лет распространенность кариеса составила 81,8–93,0%, у детей группы сравнения – 79,2%. При этом значения индекса КПУ+кп у воспитанников интернатов варьировали от 4,45±0,64 до 6,25±0,72, в то время как у школьников они составили 3,56±0,18. У 12-летних детей в основной группе распространенность кариеса составляла 77,6–92,3%, в группе сравнения – 72,3%, а индекс КПУ – от 2,80±0,40 до 6,16±0,32 и 1,54±0,28 соответственно. В структуре индексных показателей у воспитанников учреждений интернатного типа всех возрастов преобладали нелеченые кариозные зубы, тогда как у школьников, воспитывающихся в семьях, большую долю составляли пломбированные зубы. У детей-сирот и детей, оставшихся без попечения родителей значения pH смешанной слюны были ниже, чем у детей, воспитывающихся в семьях, а более высокие баллы ТЭР-теста соответствовали меньшей кариесрезистентности эмали. Заключение. У детей, проживающих в учреждениях интернатного типа, распространенность и интенсивность кариеса были выше, чем у школьников, воспитывающихся в семьях. В структуре индексных показателей у этих детей также преобладали нелеченые кариозные зубы, а показатели кариесрезистентности эмали были менее благоприятными. Полученные данные свидетельствуют о более высокой потребности воспитанников интернатных учреждений в своевременной санации полости рта и дифференцированном профилактическом наблюдении.</p></abstract><trans-abstract xml:lang="en"><p>Objective. To compare dental caries prevalence and intensity, enamel caries resistance, and mixed-saliva pH in children aged 6–7 and 12 years raised in families or residential care institutions. Materials and methods. Baseline data from 484 children aged 6–7 and 12 years were analysed: 287 residents of three residential care institutions and 197 schoolchildren raised in families. In children aged 6–7 years, caries intensity was assessed using the combined DMFT+dft index; in 12-year-olds, using the DMFT index. Caries prevalence was calculated as the proportion of children with at least one decayed, filled, or missing tooth due to caries. Index structure, pH of mixed saliva, and enamel resistance test scores were also analysed. Results. In 6–7-year-old residents, caries prevalence was 81.8–93.0% versus 79.2% in the comparison group; DMFT+dft was 4.45±0.64 to 6.25±0.72 versus 3.56±0.18. In 12-year-old residents, prevalence was 77.6–92.3% versus 72.3%; DMFT was 2.80±0.40 to 6.16±0.32 versus 1.54±0.28. Untreated decayed teeth predominated among residents, whereas filled teeth formed the larger component in the comparison group. Residents had lower mixed-saliva pH and higher enamel resistance test scores, indicating lower enamel caries resistance. Conclusion. Children raised in residential care institutions had higher caries prevalence and intensity, a higher proportion of untreated decayed teeth in the corresponding caries indices, and less favourable enamel caries-resistance indicators than schoolchildren raised in families.
