Параллельно были обновлены и рекомендации американских профессиональных ассоциаций (AHA/ACC), что позволяет провести сравнительный анализ ключевых направлений в мировой реаниматологии. Ключевые европейские изменения включают переход на стратегию поддержания целевых физиологических показателей (EtCO2, артериальное давление) во время сердечно – легочной реанимации (СЛР), уточнение сроков введения адреналина при шоковых ритмах, рекомендацию использовать максимальную доступную энергию для дефибрилляции, а также смещение в постреанимационной фазе от целевого управления гипотермией (TTM 32-34°C) к активному предупреждению лихорадки (≤37.5°C) [<xref ref-type="bibr" rid="cit3">3</xref>]. Американские рекомендации, в свою очередь, делают значительный акцент на качестве СЛР, минимизации перерывов, более точном определении моментов для прекращения реанимации и отказе от устаревших методик [<xref ref-type="bibr" rid="cit2">2</xref>]. Данный обзор детально представляет эти новации в сравнительном ключе, интегрируя их в структурированный клинический контекст для практикующих специалистов.</p></abstract><trans-abstract xml:lang="en"><p>In 2025, the European Resuscitation Council (ERC) released updated Adult Advanced Life Support (ALS) guidelines, marking a paradigm shift from rigid algorithms to a dynamic “Physiologically-focused CPR” concept. Simultaneously, American Heart Association/American College of Cardiology (AHA/ ACC) guidelines have been updated, allowing for a comparative analysis of key directions in global resuscitation science. Key European updates include a move towards maintaining target physiological parameters (EtCO2, blood pressure) during CPR, refined timing of adrenaline administration for shockable rhythms, a recommendation to use the maximum available energy for defibrillation, and a post-resuscitation care shift from targeted hypothermia management (TTM 32-34°C) to active fever prevention (≤37.5°C) [<xref ref-type="bibr" rid="cit3">3</xref>]. The American guidelines, in turn, place significant emphasis on CPR quality, minimizing interruptions, more precise determination of when to terminate resuscitation, and the abandonment of outdated procedures [<xref ref-type="bibr" rid="cit2">2</xref>].
