Приведено описание клинического случая, иллюстрирующего типичный комплекс диагностических трудностей при СЛИ: начальное ошибочное отнесение симптоматики к миастении гравис, отсутствие эффекта от антихолинэстеразной терапии, необходимость целенаправленного выявления феномена «врабатывания» и опоры на данные высокочастотной ритмической стимуляции в комплексной оценке электрофизиологической картины. Подчеркнуто, что подозрение на паранеопластический СЛИ позволяет проводить настойчивый, повторный онкопоиск даже при отрицательных первичных результатах визуализации, поскольку неврологическая симптоматика может манифестировать за несколько месяцев до верификации опухолевого процесса.</p></abstract><trans-abstract xml:lang="en"><p>The article presents a review of the current literature on paraneoplastic Lambert-Eaton syndrome (LES). A case report is provided, illustrating typical diagnostic challenges: initial misclassiﬁcation as myasthenia gravis, lack of response to acetylcholinesterase therapy, the need to identify the «warm-up» phenomenon and rely on high-frequency repetitive nerve stimulation in the comprehensive electrophysiologic assessment of LES.
