Ладонно-подошвенный псориаз является социально значимой формой хронического дерматоза, приводящей к выраженному снижению качества жизни и ограничению трудовой деятельности. Поиск эффективных и безопасных методов лечения, особенно при наличии противопоказаний к системной терапии, сохраняет свою актуальность.

Цель

Цель. Оценить клиническую эффективность и переносимость применения импульсного лазера на красителях у пациентов с ладонноподошвенным псориазом.

Материал и методы

Материал и методы. Приведено описание двух клинических случаев. Первый пациент (61 год) получал комбинацию топической терапии (салициловая кислота, бетаметазон) и курса импульсного лазера на красителях. Второй случай (пациентка с контролируемым сахарным диабетом 2 типа и непереносимостью метотрексата) лечился импульсным лазером на красителях после отмены цитостатика. Эффективность оценивали по динамике индекса тяжести ладонно-подошвенного псориаза (PPASI).

Результаты

Результаты. У обоих пациентов отмечена выраженная положительная динамика. В первом случае индекс PPASI снизился с 18 баллов до 1 балла. Во втором случае – с 15 баллов до 0,5 балла. Побочные эффекты фототерапии отсутствовали.

Заключение

Заключение. Импульсный лазер на красителях продемонстрировал высокую эффективность и хорошую переносимость в лечении ладонно-подошвенного псориаза. Метод может рассматриваться как оптимальный вариант терапии, особенно при наличии коморбидной патологии, ограничивающей применение системных препаратов.

Palmoplantar psoriasis is a socially significant form of chronic dermatosis leading to a marked decrease in the quality of life and limitation of work activity. The search for effective and safe treatment methods, especially in the presence of contraindications to systemic therapy, remains relevant.

Objective

Objective. To evaluate the clinical efficacy and tolerability of pulsed dye laser in patients with palmoplantar psoriasis.

Material and methods

Material and methods. Two clinical cases are described. The first patient (61 years old) received a combination of topical therapy (salicylic acid, betamethasone) and a course of pulsed dye laser. The second case (a patient with controlled type 2 diabetes mellitus and intolerance to methotrexate) was treated with pulsed dye laser after discontinuation of the cytostatic agent. Efficacy was assessed by the dynamics of the palmoplantar psoriasis severity index (PPASI).

Results

Results. Both patients showed significant positive dynamics. In the first case, the PPASI score decreased from 18 points to 1 point. In the second case, it decreased from 15 points to 0.5 points. There were no side effects from phototherapy.

Conclusion

Conclusion. Pulsed dye laser demonstrated high efficacy and good tolerability in the treatment of palmoplantar psoriasis. This method can be considered an optimal treatment option, especially in the presence of comorbid conditions that limit the use of systemic medications.
