Разработать и оценить эффективность комплексного подхода к коррекции цитокинового статуса крови пациентов с псориатическим артритом (ПсА) на фоне метаболического синдрома (Мс), включающего тофацитиниб (Тофа) и физиотерапевтические методики.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включено 230 пациентов с ПсА и Мс, рандомизированных на 5 групп. Все пациенты получали стандартную медикаментозную терапию (Ст) ПсА. В 1-й группе назначалась только Ст, во 2-й – дополнительно 30 процедур надвенного лазерного облучения крови (НЛОК+Ст), в 3-й – дополнительно 30 сеансов фототерапии спектра 311 нм (Фт+Ст), в 4-й – тофацитиниб по 10 мг 2 раза в день в течение 6 месяцев (Тофа+Ст), в 5-й – комплекс НЛОК+Фт+Тофа+Ст. В крови определяли уровни интерлейкинов (ИЛ) – 1β, -2, -4, -6, -8, -10, -13, -17, -18, -23, ФНО-α и ИНФ-γ методом иммуноферментного анализа.</p></sec><sec><title>Результаты</title><p>Результаты. Проведение Ст терапии не оказало влияния на дисбаланс в системе про- и противовоспалительных цитокинов крови. Наибольший корригирующий эффект отмечен в группе НЛОК+Фт+Тофа+Ст, где имело место снижение в крови ИЛ-1β – на 72,2%, ИЛ-2 – на 67%, ИЛ-6 – на 79,5%, ИЛ-8 – на 80,6%, ИЛ-13 – на 51,2%, ИЛ-17 – на 64,7%, ИЛ-18 – на 40,1% ИЛ-23 – на 55,9%, ФНО-α – на 77,4%, ИНФ-γ – на 61,2% и повышение уровней ИЛ-4 на 73,2%, ИЛ-10 – на 123,5%.</p></sec><sec><title>Заключение</title><p>Заключение. Эффективное корригирующее действие на цитокиновый статус крови пациентов с ПсА и Мс оказала программа комплексного лечения с дополнительным применением НЛОК, Фт и Тофа, что проявилось в снижении исходных уровней провоспалительных цитокинов на 40,1%-80,6% и повышении уровней противовоспалительных ИЛ-4 и ИЛ-10 на 73,2% и 123,5% соответственно.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To develop and evaluate the effectiveness of a comprehensive approach to correcting blood cytokine levels in patients with psoriatic arthritis (PsA) associated with metabolic syndrome (Ms), including tofacitinib (Tofa) and physiotherapy.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 230 patients with PsA and Ms, randomized into 5 groups. All patients received standard drug therapy (SDT) for Ps A. Group 1 received St alone, group 2 received an additional 30 sessions of intravenous laser irradiation (SVLI+St), group 3 received an additional 30 sessions of 311 nm phototherapy (Ft+St), Group 4 received tofacitinib 10 mg 2 times a day for 6 months (Tofa+St), and group 5 received a combination of SVLI+Ft+Tofa+St. Blood levels of interleukins (IL) – 1β, -2, -4, -6, -8, -10, -13, -17, -18, -23, TNF-α, and INF-γ were determined using enzyme-linked immunosorbent assay.</p></sec><sec><title>Results</title><p>Results. St therapy had no effect on the imbalance in the pro- and anti-inflammatory cytokine system in the blood. The greatest corrective effect was noted in the SVLI+Ft+Tofa+St group, where there was a decrease in the blood IL-1β by 72,2%, IL-2 by 67%, IL-6 by 79,5%, IL-8 by 80,6%, IL-13 by 51,2%, IL-17 by 64,7%, IL-18 by 40,1% IL-23 by 55,9%, TNF-α by 77,4%, INF-γ by 61,2% and an increase in the levels of IL-4 by 73,2%, IL-10 by 123,5%.</p></sec><sec><title>Conclusion</title><p>Conclusion.
