Актуальные демографические данные демонстрируют неуклонное увеличение продолжительности жизни, вследствие чего отмечается возрастание доли возраст-ассоциированных нозологий в структуре обращаемости за медицинской помощью. У женщин старше 45 лет особое место в структуре заболеваемости занимают состояния, связанные с перименопаузальным периодом. Потеря периферического сосудистого контроля, возникающая на фоне дефицита эстрогенов, приводит к дилатации дермальных и подкожных кровеносных сосудов, что может значимо ухудшать течение розацеа. Применение менопаузальной гормональной терапии (МГТ) иTECAR-терапии у пациенток перименопаузального возраста представляется перспективным методом лечения и поддержания стойкой ремиссии при эритемато-телеангиэктатическом подтипе розацеа.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. В сравнительном аспекте оценить эффективность применения TECAR-терапии и МГТ у пациенток перименопаузального возраста с эритемато-телеангиэктатическим подтипом розацеа в отношении клинических симптомов дерматоза и показателей качества жизни с учетом ближайших и отдаленных результатов наблюдений.</p></sec><sec><title>Методы</title><p>Методы. В одноцентровом открытом проспективном сравнительном рандомизированном исследовании приняли участие 63 пациентки перименопаузального возраста с ЭТПР. МГТ проводилась по назначению врача-гинеколога препаратом для непрерывной терапии, содержащим 1 мг эстрадиола и 10 мг дидрогестерона. Процедуры TECAR-терапии проводились с помощью аппарата «INDIBA Elite NS». Для оценки клинической эффективности лечения использовался модифицированный дерматологический индекс шкалы симптомов для розацеа (ДИШСР). Определение влияния терапевтических схем на качество жизни пациенток с розацеа осуществлялось с помощью дерматологического индекса качества жизни (ДИКЖ).</p></sec><sec><title>Результаты</title><p>Результаты. После завершения лечения во всех группах была отмечена значимая положительная динамика, отражающая эффективность применяемых методов (p T1-T2&lt;0,001 во всех группах), однако наиболее низкие показатели ДИШСР отмечены в группе С, где проводилось комплексная терапия с применением МГТ и TECAR-терапии. Динамика показателя ДИКЖ коррелировала с клинической эффективностью терапевтических схем – наиболее выраженное положительное влияние на качество жизни пациенток с ЭТПР оказывала комплексная схема МГТ и TECAR-терапии как в ближайшем, так и отдаленном периоде наблюдения.</p></sec><sec><title>Заключение</title><p>Заключение. Комплексное лечение ЭТПР с использованием МГТ и TECAR-терапии превосходит по эффективности монотерапию составляющими метода, что связано с синергизмом МГТ и радиочастотной терапии в отношении сосудистого компонента дерматоза, а также противовоспалительными эффектами TECAR-терапии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Current demographic data demonstrate a steady increase in life expectancy, resulting in an increasing proportion of age-related conditions in the pattern of medical care seeking. In women over 45, conditions associated with the perimenopausal period occupy a particularly prominent place in the morbidity structure. Loss of peripheral vascular control, which occurs due to estrogen deficiency, leads to dilation of dermal and subcutaneous blood vessels, which can significantly worsen the course of rosacea. The use of menopausal hormone therapy (MHT) and TECAR therapy in perimenopausal patients appears to be a promising method for treating and maintaining stable remission in the erythematotelangiectatic subtype of rosacea.</p></sec><sec><title>Study objective</title><p>Study objective. To comparatively evaluate the efficacy of TECAR therapy and MHT in perimenopausal patients with erythematotelangiectatic rosacea (ETR) in terms of clinical symptoms of the dermatosis and quality of life, taking into account short-term and long-term follow-up results.</p></sec><sec><title>Methods</title><p>Methods. A single-center, open-label, prospective, comparative, randomized study included 63 perimenopausal patients with ETR. MHT was administered as prescribed by a gynecologist using a continuous therapy preparation containing 1 mg estradiol and 10 mg dydrogesterone. TECAR therapy procedures were performed using the INDIBA Elite NS device. The modified dermatological index of the rosacea symptom scale (DIRSS) was used to evaluate the clinical efficacy of treatment. The impact of treatment regimens on the quality of life of patients with rosacea was determined using the dermatological index of quality of life (DIQL).</p></sec><sec><title>Results</title><p>Results. Upon completion of treatment, significant positive dynamics were noted in all groups, reflecting the effectiveness of the methods used (p T1- T2&lt;0.001 in all groups). However, the lowest DIQL values were observed in Group C, which received combination therapy using MHT and TECAR therapy. The dynamics of the DIQL indicator correlated with the clinical effectiveness of the therapeutic regimens: the most pronounced positive effect on the quality of life of patients with ETPR was provided by the combination of MHT and TECAR therapy in both the short-term and long-term follow-up periods.</p></sec><sec><title>Conclusion</title><p>Conclusion.
