Атопический дерматит (АтД) – хроническое воспалительное заболевание кожи с многофакторным патогенезом и выраженной клинической гетерогенностью. Комплексный анализ, объединяющий демографические и анамнестические данные, объективные биомаркеры и атопические коморбидности, расширяет возможности персонализированного ведения.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Стратифицировать пациентов с атопическим дерматитом по степени тяжести на основании клиникоанамнестических данных и уровня белка филаггрина.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. в одноцентровом проспективном исследовании тяжесть заболевания определяли по индексу оценки атопического дерматита исследователем (IGA), а экспрессию филаггрина – методом тейп-стриппинга с последующим иммуногистохимическим анализом; также собирали демографические характеристики, данные об атопических коморбидностях и семейном атопическом анамнезе.</p></sec><sec><title>Результаты</title><p>Результаты. Включено 90 пациентов; преобладали средняя и тяжелая степени тяжести (36/90 и 34/90). Дебют в детском возрасте чаще встречался при IGA 3/4 (в тяжелой группе 30/34; в средней 14/36). Уровень филаггрина различался между группами: медиана 0,171 [0,107; 0,254] при тяжелой и 0,499 [0,339; 1,038] при средней степени; при IGA 1–2 значения были выше (0,415 [0,372; 0,458] и 0,433 [0,191; 0,618]).</p></sec><sec><title>Заключение</title><p>Заключение. Фенотипический профиль пациентов с АтД существенно меняется по мере утяжеления заболевания и связан с возрастом дебюта, атопическими коморбидностями и уровнем барьерного маркера филаггрина. Неинвазивная оценка филаггрина методом тейп-стриппинга показала технологическую простоту и клиническую сопоставимость с тяжестью, что позволяет рассматривать ее как дополнительный инструмент мониторинга в клинической практике.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Atopic dermatitis (AD) is a chronic inflammatory skin disease with a multifactorial pathogenesis and marked clinical heterogeneity. A comprehensive assessment combining demographic and clinical-history data, objective biomarkers, and atopic comorbidities expands the possibilities for personalized care.</p></sec><sec><title>Aim</title><p>Aim. To stratify patients with atopic dermatitis according to severity based on clinical and anamnestic data and filaggrin protein levels.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. In a single-centre prospective study, disease severity was determined using the Investigator’s Global Assessment (IGA) score, and filaggrin expression was determined using tape stripping followed by immunohistochemical analysis. Demographic characteristics, data on atopic comorbidities, and family history of atopy were also collected.</p></sec><sec><title>Results</title><p>Results. Ninety patients were included; moderate and severe severity prevailed (36/90 and 34/90). Onset in childhood was more common in IGA 3/4 (30/34 in the severe group; 14/36 in the moderate group). Filaggrin levels differed between groups: median 0.171 [0.107; 0.254] in the severe group and 0.499 [0.339; 1.038] in the moderate group; in IGA 1–2, the values were higher (0.415 [0.372; 0.458] and 0.433 [0.191; 0.618]).</p></sec><sec><title>Conclusion</title><p>Conclusion. The phenotypic profile of patients with AD changes significantly as the disease worsens and is associated with age of onset, atopic comorbidities, and filaggrin barrier marker levels.
