Проведено проспективное рандомизированное исследование (2021–2025 гг.) с участием 168 пациентов с гистологически подтвержденным IV стадией рака желудка, получающих иммунотерапию ингибиторами PD-1 (пембролизумаб/ ниволумаб) в 3–4 линиях терапии. Все пациенты были распределены на две группы: экспериментальную (n=82) – иммунотерапия + комплексная реабилитация (нутритивная поддержка, H2/O2-ингаляции, магнитотерапия, психологическая коррекция, коррекция водно-электролитного баланса); контрольную (n=86) – только иммунотерапия. Особое внимание уделено подгруппе пациентов с радикальной гастрэктомией в анамнезе (n=84). Расчет необходимого объема выборки. Априорный расчет мощности исследования был выполнен на основе первичного исхода – частоты подтвержденного прогрессирования (iCPD) среди пациентов с неподтвержденным прогрессированием (iUPD) на 8-й неделе терапии. Согласно данным пилотных исследований и литературным источникам [12,13], ожидаемая частота iCPD в контрольной группе составляла 70–75 %. С учетом клинической значимости снижения iCPD до 45 % в экспериментальной группе, при двустороннем тесте, уровне значимости α = 0,05 и статистической мощности 1–β = 0,80, необходимый объем каждой подгруппы (пациенты с гастрэктомией) составил не менее 38 человек. С учетом возможных потерь (до 10 %) и необходимости обеспечения сопоставимости по основным прогностическим факторам, планируемый размер подгруппы был увеличен до 42 пациентов на группу, что в сумме дало целевую выборку из 84 пациентов с гастрэктомией. Общий объем исследования (n = 168) включал также равное количество пациентов без гастрэктомии для межгруппового сравнения базовых характеристик и общей репрезентативности когорты. Расчет выполнен с использованием программного обеспечения G*Power 3.1.</p></sec><sec><title>Результаты</title><p>Результаты. Пациенты с гастрэктомией в экспериментальной группе демонстрировали выраженный пострезекционный синдром: исходный PNI 34,66±2,8 (vs 38,15±3,5 у без резекции, p=0,0047), гипокальциемия (2,05±0,15 ммоль/л), гипомагниемия (0,61±0,09 ммоль/л), хроническая боль (78,6 %). Через 6 месяцев в экспериментальной группе отмечены: максимальный прирост PNI (+21,54 до 56,20, p&lt;0,001), нормализация азотистого баланса (+5,2 г/сут, p=0,03), повышение Ca²+ (до 2,31 ммоль/л) и Mg²+ (до 0,87 ммоль/л) (p&lt;0,05), значимое улучшение качества жизни по всем доменам EORTC QLQ-C 30 (общее КЖ +15,2 %, физическое функционирование +14,8 %, боль –55,6 %, p&lt;0,05), снижение приверженности к терапии по психологическим причинам в 3,2 раза (ОР=0,31). Частота подтвержденного прогрессирования (iCPD) у пациентов с неподтвержденным прогрессированием (iUPD) на 8-й неделе была достоверно ниже в экспериментальной группе (45,0 % против 72,3 %, p=0,05).</p></sec><sec><title>Выводы</title><p>Выводы. Пациенты с гастрэктомией в анамнезе формируют уязвимую подгруппу с выраженным пострезекционным синдромом мальабсорбции, электролитной недостаточности и нутритивной дезадаптации, что ухудшает переносимость и потенциально снижает эффективность иммунотерапии. Комплексная реабилитация, интегрирующая метаболическую, нутритивную, физическую и психологическую поддержку, достоверно восстанавливает гомеостаз, улучшает качество жизни и способствует стабилизации заболевания. Полученные данные обосновывают необходимость раннего включения персонализированных реабилитационных программ в стандарты ведения данной категории пациентов.</p></sec></abstract><trans-abstract xml:lang="en"><p>Aim of the study – to comprehensively evaluate the impact of postresection syndrome on the tolerability and efficacy of immunotherapy in patients with metastatic gastric cancer and to determine the role of personalized comprehensive rehabilitation in correcting metabolic-nutritional, electrolyte, and psychological impairments in this vulnerable subgroup.</p><sec><title>Materials and methods</title><p>Materials and methods. A prospective randomized study (2021–2025) included 168 patients with histologically confirmed stage IV gastric cancer receiving PD-1 inhibitor immunotherapy (pembrolizumab/nivolumab) in the 3rd-4th line. Patients were randomized into an experimental group (n=82): immunotherapy + comprehensive rehabilitation (nutritional support, H2/O2 inhalations, magnetic therapy, psychological correction, electrolyte balance correction); and a control group (n=86): immunotherapy only. Special focus was placed on the subgroup with prior total gastrectomy (n=84).</p></sec><sec><title>Results</title><p>Results. Gastrectomized patients in the experimental group exhibited a pronounced postresection syndrome: baseline PNI 34.66±2.8 (vs. 38.15±3.5 in non-resected, p=0.0047), hypocalcemia (2.05±0.15 mmol/L), hypomagnesemia (0.61±0.09 mmol/L), and chronic pain (78.6 %). After 6 months, the experimental group demonstrated: maximal PNI increase (+21.54 to 56.20, p&lt;0.001), nitrogen balance normalization (+5.2 g/day, p=0.03), Ca²+ elevation (to 2.31 mmol/L) and Mg²+ recovery (to 0.87 mmol/L) (p&lt;0.05), and significant EORTC QLQ-C 30 improvement (global QoL +15.2 %, physical functioning +14.8 %, pain reduction –55.6 %, p&lt;0.05). Psychological reasons for therapy discontinuation decreased 3.2-fold (OR=0.31). Confirmed progression (iCPD) among patients with unconfirmed progression (iUPD) at week 8 was significantly lower in the experimental group (45.0 % vs. 72.3 %, p=0.05).</p></sec><sec><title>Conclusions</title><p>Conclusions. Patients with a history of gastrectomy constitute a vulnerable subgroup with severe postresection malabsorption, electrolyte deficiency, and nutritional decompensation that impair immunotherapy tolerability and potentially reduce efficacy. Comprehensive rehabilitation integrating metabolic, nutritional, physical, and psychological support effectively restores homeostasis, improves quality of life, and promotes disease stabilization.
