В последние десятилетия удалось существенно повысить пятилетнюю выживаемость детей, перенесших острый лимфобластный лейкоз (ОЛЛ), которая в России превышает 90 %. Вместе с тем у данной группы пациентов сохраняются серьезные риски развития отдаленных эффектов терапии, включая вторичные злокачественные новообразования, кардиотоксичность, нейрокогнитивные нарушения, а также патологии опорно-двигательного аппарата и эндокринной системы. Основными факторами риска поздних эффектов являются: интенсивность и комбинация химио /лучевой терапии, возраст на момент лечения, генетическая предрасположенность (полиморфизмы генов, вариации генов ДНК-репарации), а также наличие сопутствующих заболеваний (врожденные патологии сердца, ожирение).Цель. Систематизация и обобщение современных данных о спектре и частоте поздних неблагоприятных эффектов у взрослых, излеченных от ОЛЛ в детском возрасте.Материалы и методы. Проведен структурированный обзор и анализ литературных данных в электронных медицинских базах данных PubMed, eLibrary за период с 1990 по 2024 г.Заключение. Данные обзора подчеркивают высокую распространенность разнообразных поздних неблагоприятных эффектов у детей, излеченных от ОЛЛ, и необходимость их систематического мониторинга.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. In recent decades, the five-year survival rate of children with acute lymphoblastic leukemia (ALL) in Russia has risen to over 90 %. At the same time, this group of patients remains at serious risk of developing long-term effects of therapy, including secondary malignant neoplasms, cardiotoxicity, neurocognitive disorders, as well as pathologies of the musculoskeletal apparatus and endocrine system. The main risk factors for late effects are: intensity and combination of chemotherapy/radiation therapy, age at the time of treatment, genetic predisposition (gene polymorphisms, variations in DNA repair genes), as well as the presence of concomitant diseases (congenital heart disease, obesity).Objective. To systematize and summarize current data on the spectrum and frequency of late adverse effects in patients treated for ALL during childhood.Materials and methods. Data were structured review and analyzed in electronic medical databases such as PubMed, eLibrary from 1990 to 2024.Conclusions.
