Оценить частоту гипертонической реакции артериального давления (АД) на физическую нагрузку (ГРН) при проведении стресс-эхокардиографии (СЭ) у пациентов с известной или подозреваемой ишемической болезнью сердца и влияние ГРН на результаты ABCDE-СЭ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В одноцентровое исследование включено 193 пациента, которым была выполнена ABCDE-СЭ (оценка нарушений локальной сократимости, В-линий, резервов сократительного (СР), коронарного, хронотропного) с традиционной оценкой реакции АД. Были изучены частота возникновения ГРН в ходе СЭ и взаимосвязь ГРН с положительными результатами СЭ.</p></sec><sec><title>Результаты</title><p>Результаты. Распространенность ГРН составила 36,3% (70 пациентов). Пациенты с ГРН чаще имели низкую толерантность к физической нагрузке (р=0,013); скорость кровотока в коронарной артерии в покое и на нагрузке, величина СР были выше у пациентов с ГРН (р=0,009, р=0,008 и р=0,002). Среди пациентов с ГРН снижение СР встречалось реже, чем среди пациентов с адекватной реакцией АД: 45 (64,3%) против 99 (80,5%) (р=0,013).</p></sec><sec><title>Заключение</title><p>Заключение. Чрезмерное повышение АД во время СЭ может быть ограничивающим признаком, приводящим к преждевременному прерыванию тестирования, что означает худшую переносимость физической нагрузки по сравнению с нормотензивными пациентами. Имеется взаимосвязь между реакцией АД на нагрузку и результатами ABCDE-СЭ, в частности влияние пикового АД на СР. Взаимосвязь между реакцией АД на нагрузку и шагами СЭ, а также ее прогностическая значимость требуют дальнейшего изучения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To assess the incidence of hypertensive response of arterial pressure (BP) to exercise (HRE) during stress echocardiography (SE) in patients with known or suspected coronary artery disease and the effect of HRE on the results of ABCDE-SE.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The single-center study included 193 patients who underwent ABCDE-SE (evaluation of wall motion abnormalities, B-lines, contractile reserve (CR), coronary, chronotropic) with traditional assessment of BP response. The incidence of HRE during SE and the relationship of HRe with positive SE results were studied.</p></sec><sec><title>Results</title><p>Results. The prevalence of HRE was 36.3% (70 patients). Patients with HRE more often had low exercise tolerance (p=0.013); coronary artery blood flow velocity at rest and under load, CR value were higher in patients with HRE (p=0.009, p=0.008 and p=0.002). Among patients with HRE CR decrease was less common than among patients with adequate BP response: 45 (64.3%) versus 99 (80.5%) (p=0.013).</p></sec><sec><title>Conclusion</title><p>Conclusion. Excessive BP increase during SE may be a limiting feature leading to premature termination of testing, which means worse exercise tolerance compared to normotensive patients. There is a relationship between BP response to load and ABCDE-SE results, in particular the effect of peak BP on CR.
