Успех ортодонтической коррекции на несъемных аппаратах во многом зависит от стабильности каждого элемента на поверхности эмали в течение всего курса лечения. Брекет не должен затруднять гигиену полости рта пациентов на ортодонтическом лечении, выдерживать окклюзионные нагрузки, жевание и тяговые силы, создаваемые дугой. Кроме того, брекет должен сниматься после лечения без повреждения эмали. Такие переменные как тип полимеризации, используемые адгезивы, способы подготовки эмали перед фиксацией влияют на адгезионную прочность брекета. Современный научный поиск ведется в направлении улучшения свойств адгезии бондов и является актуальной задачей. Обработка гипохлоритом натрия способствует удалению деминерализованного коллагена с поверхности зуба. В свою очередь роль фосфорной кислоты заключается в усилении прочности на сдвиг при фиксации брекетов. В данной статье проводится оценка прочности сцепления при сдвиге адгезивной системы с предварительной обработкой гипохлоритом натрия и ортофосфорной кислотой в лабораторных условиях.</p></sec><sec><title>Цель</title><p>Цель: изучить прочность сцепления на сдвиг универсальной адгезивной системы в лабораторных условиях при фиксации брекетов с различными способами подготовки поверхности эмали.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Для исследования подготовлено n=60 (100%) образцов зубов, удаленных по ортодонтическим показаниям, без кариозных повреждений и больших пломб. Каждый зуб помещали в блок с самотвердеющей пластмассой или гипсом. Образцы были разделены на три группы по n=20 образцов. Перед нанесением адгезивной системы поверхность образцов n=20 (33.3%) 1 группы обрабатывали 37% ортофосфорной кислотой, n=20 (33.3%) образцов 2 группы – 3,25% гипохлоритом натрия, в 3 группе n=20 (33.3%) образцов были без предварительной обработки. Затем на подготовленные образцы фиксировали металлические брекет-системы. Каждая из групп делилась на 2 подгруппы – n=10 образцов с термоциклированием и n=10 образцов без термоциклирования. Для фиксации брекетов применялись двухкомпонентная самоотверждаемая адгезивная система Universal Bond II (Tokuyama Dental); адгезивный фиксирующий цемент двойного отверждения ESTECEM II PLUS (Tokuyama Dental), 37% ортофосфорная кислота и 3,25% гипохлорит натрия.</p></sec><sec><title>Результаты</title><p>Результаты: наблюдалось статистически значимое различие между группами с предварительной обработкой 37% ортофосфорной кислотой и 3,25% гипохлоритом натрия, и группой без предварительной обработки, как до термоциклирования, так и после. Самые высокие значения прочности сцепления на сдвиг до термоциклирования были получены в группе с предварительной обработкой фосфорной кисло- той (10,12±1,27 мПа). Самая слабая прочность на сдвиг (4,10±0,96 мПа) обнаружена в образцах без предварительной обработки эмали. После термоциклирования результаты адгезионной прочности достоверно снижались.</p></sec><sec><title>Выводы</title><p>Выводы. Применение 37% ортофосфорной кислоты и 3,25% гипохлорита натрия может повышать прочность сцепления при сдвиге брекет-систем с эмалью более чем в два раза. Термоциклирование снижает прочность сцепления при сдвиге во всех группах. Однако, имеющихся исследований недостаточно для оценки роли 37% ортофосфорной кислоты и 3,25% гипохлорита натрия в прочности сцепления при сдвиге, в связи с чем необходимы дальнейшие исследования в этой области.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. The success of orthodontic correction on fixed devices largely depends on the stability of each element on the enamel surface during the entire course of treatment. Braces should not interfere with oral hygiene of patients undergoing orthodontic treatment, and should withstand occlusive loads, chewing, and traction forces generated by the arch. In addition, the braces should be removed after treatment without damaging the enamel. Variables such as the type of polymerization, the adhesives used, and the methods of preparing the enamel before fixation affect the adhesive strength of the bracket. Modern scientific research is being conducted in the direction of improving the adhesion properties of bonds and is an urgent task. Treatment with sodium hypochlorite helps to remove demineralized collagen from the tooth surface. In turn, the role of phosphoric acid is to enhance the shear strength when fixing braces. This article evaluates the shear strength of an adhesive system with pretreatment with sodium hypochlorite and orthophosphoric acid in laboratory conditions.</p></sec><sec><title>Aims</title><p>Aims: to study the shear strength of a universal adhesive system in the laboratory when fixing braces with various methods of enamel surface preparation.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The samples of teeth removed according to orthodontic indications n=60 (100%), without carious lesions and large fillings, were prepared for the study. Each tooth was placed in a block with self-hardening plastic or plaster. The samples were divided into three groups of n=20 samples. Before applying the adhesive system, the surfaces of n=20 (33.3%) samples were treated with 37% phosphoric acid, n=20 (33.3%) other samples were treated with 3.25% sodium hypochlorite, n=20 (33.3%) samples were without pretreatment. Then metal braces were fixed on the prepared samples. Each of the groups was divided into 2 subgroups – n=10 samples with thermal cycling and n=10 samples without thermal cycling. Two-component self-curing adhesive system Universal Bond II (Tokuyama Dental); adhesive fixing cement ESTECEM II PLUS (Tokuyama Dental), 37% orthophosphoric acid and 3.25% sodium hypochlorite were used to fix the braces.</p></sec><sec><title>Results</title><p>Results: there was a statistically significant difference between the groups with 37% phosphoric acid and 3.25% sodium hypochlorite pretreatment and the group without pretreatment, both before and after thermal cycling. The highest shear strength values before thermal cycling were obtained in the group with phosphoric acid pretreatment (10.12±1.27 MPa). The weakest shear strength (4.10±0.96 MPa) was found in samples without pretreatment of the enamel. After thermal cycling, the results of adhesive strength significantly decreased.</p></sec><sec><title>Conclusions</title><p>Conclusions. The use of 37% orthophosphoric acid and 3.25% sodium hypochlorite can increase the adhesion strength of braces to enamel by more than two times. Thermal cycling reduces shear strength in all groups.
