В представленном исследовании изучалась сила четырехглавой мышцы бедра при различных вариантах регионарной анальгезии после эндопротезирования тазобедренного сустава (ЭТС).</p></sec><sec><title>Цель</title><p>Цель. Сравнить силу четырехглавой мышцы бедра при использовании илеофасциальной (ИФБ) или перикапсульной блокады группы нервов (ПБГН) в схеме мультимодальной анальгезии после ЭТС.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование было включено 50 пациентов, которым планировалось выполнение первичного ЭТС. Больные были рандомизированы в две группы в зависимости от вида регионарной анестезии. Больным 1-й группы (n=25) –выполняли ИФБ, а больным 2-й группы – ПБГН. В послеоперационном периоде проводилась оценка силы четырехглавой мышцы бедра при помощи шкалы мышечной силы через 3–6 – 8–12–18–24 часа после операции. Дополнительно оценивали время первой вертикализации, уровень болевого синдрома в покое и при движении через 3–6 – 12–24 часа с использованием цифровой-рейтинговой шкалы. Регистрировали потребность в назначении наркотических анальгетиков в первые сутки после операции.</p></sec><sec><title>Результаты</title><p>Результаты. Пациенты с ПБГН имели достоверно большие показатели силы четырехглавой мышцы бедра через 3, 6 и 8 часов после операции, при сравнении с пациентами с ИФБ (p &lt;0,05). Через 12, 18 и 24 часа статистически значимых различий в данном показателе между группами отмечено не было. Время первой вертикализации было меньше у пациентов 2 группы. Уровень болевого синдрома в покое и при движении был достоверно ниже у пациентов 2 группы через 12 часов после операции. В остальные периоды различий не отмечено. Количество назначаемых опиоидных анальгетиков в первые сутки было достоверно ниже в группе ПБГН, в сравнении с группой ИФБ.</p></sec><sec><title>Заключение</title><p>Заключение. Использование ИФБ и ПБГН в схеме мультимодальной анальгезии позволяет эффективно контролировать показатели болевого синдрома в послеоперационном периоде после ЭТС. Применение ПБГН дает возможность проводить раннюю активизацию и двигательную активность пациентов за счет мотор-сберегающего эффекта на четырехглавую мышцу бедра, что является одним из наиболее важных моментов профилактики развития осложнений и положительного исхода хирургического лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The presented study examined the strength of the quadriceps femoris muscle with different types of regional analgesia after total hip arthroplasty (THA).</p></sec><sec><title>Purpose</title><p>Purpose. To evaluate the strength of the quadriceps femoris muscle using ileofascial (IFB) or pericapsular nerve group block (PENG) in the multimodal analgesia after THA.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 50 patients who were scheduled to undergo primary THA. Patients were randomly divided into two groups: group 1 (n=25) – patients who underwent IFB, group 2 (n=25) – patients who underwent PENG. In the postoperative period, the strength of the quadriceps femoris muscle was assessed using a muscle strength scale at 3–6–8–12–18–24 hours after surgery. The time of the first verticalization was analyzed. The level of pain syndrome at rest and during movement was recorded after 3–6–12–24 hours using a digital rating scale. The need for narcotic analgesics on the first day after surgery was noted.</p></sec><sec><title>Results</title><p>Results. Patients with PENG had significantly higher quadriceps strength indicators after 3, 6 and 8 hours after surgery, when compared with patients with IFB (p &lt;0,05). After 12, 18 and 24 hours, statistically significant differences in this indicator were not noted between the groups. The time of the first verticalization was shorter in patients of group 2. The level of pain syndrome at rest and during movement was significantly lower in patients of group 2 12 hours after surgery. No differences were noted in other periods. The number of opioid analgesics prescribed on the first day was significantly lower in the PENG group, compared with the IFB group.</p></sec><sec><title>Conclusions</title><p>Conclusions. The use of IFB and PENG in multimodal analgesia allows for effective control of pain syndrome indices in the postoperative period after ETS.
