Хирургическое лечение часто требует последующей реконструкции, однако использование имплантов может приводить к рубцовым деформациям, дефициту мягких тканей и асимметрии. Перспективным методом коррекции этих осложнений является аутологичная трансплантация жировой ткани (АТЖТ), обогащенная стромально-васкулярной фракцией (СВЖФ).</p><sec><title>   Цель</title><p>   Цель. Оценка эффективности АТЖТ с СВЖФ в улучшении качества рубцов, коррекции дефектов и сохранности объема трансплантата при гибридной реконструкции молочной железы.</p></sec><sec><title>   Материалы и методы</title><p>   Материалы и методы. В работу включены 38 больных I–III стадий раком молочной железы, находившиеся на обследовании и лечении в Университетской клинической больнице № 4 (ФГАОУ ВО «Первый МГМУ им. И. М. Сеченова» Минздрава России, Москва). После проведенного хирургического лечения (радикальная мастэктомия или радикальная резекция) пациентки разделены на группу СВЖФ (n = 18) и контрольную группу (n = 20). Оценка проводилась через 3, 6 и 12 месяцев по критериям: толщина жирового слоя, качество рубцов (шкала POSAS) и коррекция асимметрии.</p></sec><sec><title>   Результаты</title><p>   Результаты. В группе СВЖФ сохранность объема жира через 6 месяцев составила 1,4 ± 0,2 см против 1,0 ± 0,2 см в контрольной группе (p &lt; 0,05). Осложнений не отмечено.</p></sec><sec><title>   Заключение</title><p>   Заключение. АТЖТ, обогащенная СВЖФ, улучшает долговременные результаты реконструкции, что подтверждает ее роль в качестве адъювантного метода.</p></sec></abstract><trans-abstract xml:lang="en"><p>   Breast cancer remains the most prevalent oncological disease among women. Surgical treatment often requires subsequent reconstruction, but implant-based approaches can lead to scar deformities, soft tissue deficiency, and asymmetry. Autologous fat grafting enhanced with stromal vascular fraction represents a promising solution for correcting these complications.</p><sec><title>   Objective</title><p>   Objective. This study aimed to evaluate the efficacy of stromal vascular fraction-enriched autologous fat grafting in improving scar quality, correcting tissue defects, and maintaining graft volume in hybrid breast reconstruction.</p></sec><sec><title>   Materials and methods</title><p>   Materials and methods. The study enrolled 38 patients with stage I–III breast cancer who were evaluated and treated at University Clinical Hospital No. 4 (Sechenov First Moscow State Medical University, Ministry of Health of Russia, Moscow). Following surgical intervention (radical mastectomy or breast-conserving surgery with radical resection). Participants were divided into a stromal vascular fraction group (n = 18) and control group (n = 20). Evaluations at 3, 6, and 12 months assessed adipose layer thickness, scar quality using the POSAS scale, and degree of asymmetry correction.</p></sec><sec><title>   Results</title><p>   Results. The stromal vascular fraction group demonstrated significantly better fat graft retention at 6 months (1.4 ± 0.2 cm versus 1.0 ± 0.2 cm in controls, p &lt; 0.05).
