Блокада правой ножки пучка Гиса (БПНПГ) имеет характерные проявления на ЭКГ, однако, в клинической практике при ее сочетании с другой патологией – инфаркт миокарда, аритмогенная дисплазия правого желудочка, сочетанное поражение проводящей системы и т.д. в интерпретации могут возникать сложности.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Разработка критериев БПНПГ на основе трехмерного векторного анализа с оценкой скорости формирования пространственной векторной петли.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В основную группу вошли 64 пациента с БПНПГ, среди которых 42 мужчин и 22 женщины, средний возраст 31,1±9,62 (М±σ) лет. Выделены пациенты с полной и неполной БПНПГ по принятым ЭКГ критериям. В контрольную группу включены 80 здоровых пациентов, средний возраст 31,5±6,3 (М±σ) лет, из них 54 женщин и 26 мужчин. У всех пациентов была проведена синхронная запись 12 стандартных отведений с последующей реконструкцией трехмерной ВКГ с помощью программного обеспечения EasyECG Rest ATES Medica (Россия).</p><p>Результаты исследования и выводы. Определены критерии БПНПГ на основе трехмерной ВКГ с анализом скорости трассировки пространственной векторной петли, которые сохраняют свою эффективность при сочетании с блокадой передневерхней ветви левой ножки пучка Гиса, а также позволяют проводить дифференциальную диагностику с инфарктами миокарда, с эпсилон волной при АДПЖ и другими заболеваниями сердечно-сосудистой системы.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Right bundle branch block (RBBB) has characteristic manifestations on an ECG, however, in clinical practice, when combined with another pathology – myocardial infarction, arrhythmogenic right ventricular dysplasia, combined damage to the conduction system, etc. difficulties may arise in interpretation.</p></sec><sec><title>The purpose of the study</title><p>The purpose of the study. Development of RBBB criteria based on three-dimensional vector analysis with an assessment of the rate of formation of a spatial vector loop.</p></sec><sec><title>Material and methods</title><p>Material and methods. The main group included 64 patients with BPH, including 42 men and 22 women, with an average age of 31.1±9.62 (M±σ) years. Patients with complete and incomplete RBBB were identified according to the accepted ECG criteria. The control group included 80 healthy patients with an average age of 31.5±6.3 (M±σ) years, 54 of them women and 26 men. Synchronous recording of 12 standard leads was performed in all patients, followed by reconstruction of a three-dimensional ECG using the EasyECG Rest ATES Medica software (Russia).</p><p>The results of the study and conclusions.
