Известно, что хроническая сердечная недостаточность у ВИЧ-инфицированных развивается в 16–54 % случаев, сопровождая развитие множественной патологии миокарда и эндотелия [<xref ref-type="bibr" rid="cit1">1</xref>]. В настоящее время современное ультразвуковое оборудование, в случае оснащения опцией спекл-трекинг, предоставляет широкие возможности в диагностике ХСН с сохраненной фракцией выброса, однако данная техника пока не является доступной во всех медицинских учреждениях, поэтому необходим поиск иных возможностей для точной диагностики ХСН с сохраненной фракцией выброса.Целью данной работы явилось обоснование использования индекса Tei как оптимального и доступного показателя для диагностики диастолической дисфункции и ХСН у лиц с ВИЧ-инфекцией. В условиях крупного стационара в течение четырех лет проводилось исследование группы больных из 240 человек, инфицированных вирусом иммунодефицита человека (ВИЧ). Исследование носило характер одномоментного, скринингового, кинического. Индекс Tei в литературе также встречается под названием «индекс производительности миокарда» [<xref ref-type="bibr" rid="cit2">2</xref>]. Клиническая значимость определения индекса Тei в определении диастолической дисфункции у больных с ВИЧ-инфекцией выше, чем соотношения Е/е’≥3, что делает этот показатель рекомендованным для эхокардиографии данной когорты пациентов, как раннего маркера диастолической дисфункции и ХСН. У ВИЧ-инфицированных повышение индекса Тei более 0,41 сопряжено с развитием диастолической дисфункции левого желудочка, гипертрофии, увеличением объема левого предсердия, легочной артериальной гипертензии, анемии и хронической болезни почек. У больных с ВИЧ-инфекцией при повышении Индекса Тei ≥,41- в 10,6 раз повышается вероятность развития ХСН, в 8 раз –тяжелой ХСН с NT-proBNP≥1500 пг/мл, в 5,75 раз – анемии, в 3,73 раза –ИБС, в 3,09 раза – хронической болезни почек; в 2,86 раза – желудочковых нарушений ритма сердца и в 2,29 раз – легочной артериальной гипертензии.</p></abstract><trans-abstract xml:lang="en"><p>Clarification of the presence of diastolic dysfunction, as a possible onset of chronic heart failure (CHF) with preserved LV ejection fraction, is strategic in determining the management tactics and prognosis of patients with HIV infection. It is known that chronic heart failure in HIV-infected people develops in 16–54 % of cases, accompanying the development of multiple pathologies of the myocardium and endothelium [<xref ref-type="bibr" rid="cit1">1</xref>]. Currently, modern ultrasound equipment, if equipped with a speckle tracking option, provides ample opportunities for diagnosing CHF with preserved ejection fraction, however, this technology is not yet available in all medical institutions, so it is necessary to search for other opportunities for accurate diagnosis of CHF with preserved ejection fraction ejectionThe purpose of this work was to substantiate the use of the Tei index as an optimal and accessible indicator for diagnosing diastolic dysfunction and CHF in people with HIV infection. In a large hospital, a group of 240 patients infected with the human immunodeficiency virus (HIV) was studied over a period of four years. The study was of a one-stage, screening, clinical nature. The Tei index is also found in the literature under the name “myocardial performance index” [<xref ref-type="bibr" rid="cit2">2</xref>]. The clinical significance of determining the Tei index in determining diastolic dysfunction in patients with HIV infection is higher than the ratio E/e’≥13, which makes this indicator recommended for echocardiography in this cohort of patients as an early marker of diastolic dysfunction and CHF.In HIV-infected patients, an increase in the Tei index of more than 0.41 is associated with the development of left ventricular diastolic dysfunction, hypertrophy, increased volume of the left atrium, pulmonary arterial hypertension, anemia and chronic kidney disease. In patients with HIV infection, with an increase in the Tei Index ≥0.41, the likelihood of developing CHF increases 10.6 times, severe CHF with NT-proBNP≥1500 pg/ml increases 8 times, anemia increases 5.75 times, 3.73 times – coronary artery disease, 3.09 times – chronic kidney disease; 2.86 times for ventricular arrhythmias and 2.29 times for pulmonary arterial hypertension.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>индекс Tei</kwd><kwd>хроническая сердечная недостаточность</kwd><kwd>ВИЧ-инфекция</kwd></kwd-group><kwd-group xml:lang="en"><kwd>Tei index</kwd><kwd>chronic heart failure</kwd><kwd>HIV infection</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Freiberg MS, Chang CH, Skanderson M et al.
