Цель. Повышение эффективности лечения пациентов с СБД ВНЧС (К07.60) на фоне гипертонуса жевательной мускулатуры (G24.8.0).

Материал и методы

Материал и методы. Согласно цели исследования сформирована группа из 10 пациентов (8 женщин и 2 мужчин в возрасте от 12 до 60 лет) с совпадением триады признаков: боль, наличие гипертонуса жевательной мускулатуры, ограничение открывания рта. Комплекс мероприятий диагностического характера реализован в объеме: клинико-рентгенологические методы обследования, МРТ ВНЧС, УЗИ жевательной мускулатуры, поверхностная ЭМГ жевательной мускулатуры, цифровая аксиография. В лечении пациентов группы исследования применялся метод инвазивной карбокситерапии, основанный на дозированных инъекциях СО2  аппаратом «INCO2» в проекции триггерных точек жевательной мускулатуры. В исследовании рассматривается терапевтическая ценность применения СО2 в протоколе функциональной реабилитации пациентов с заболеваниями ВНЧС миогенного происхождения.

Результаты

Результаты. Анализ эффективности лечения пациентов группы исследования проводился по оценке изменения динамики ключевых параметров от исходных данных: выраженность болевого синдрома по ВАШ, оценка изменения кинематики движений нижней челюсти с измерением величины открывания рта, выраженность гипертонуса ЖМ с помощью оценки величины индекса IMPACT. Выраженность болевого синдрома у пациентов группы исследования, согласно меиданным значениям, снизилась на 86% от исходных значений, определяется увеличение амплитуды открывания рта на 28,5%, снижение значений индекса IMPACT на 42,5% от исходных значений гипертонуса жевательной мускулатуры, увеличение скорости кровотока ПВА на 52,5%.

Заключение

Заключение. Результаты клинического применения метода карбокситерапии в лечении пациентов с СБД ВНЧС позволяют рекомендовать данный метод к внедрению в протоколы комплексного лечения пациентов с СБД ВНЧС с явлением гипертонуса жевательной мускулатуры. Карбокситерапия как метод этиопатогенетической реабилитации способствует скорейшему купированию болевого синдрома, нормализации биоэлектрической активности жевательной мускулатуры и восстановлению физиологической кинематики движений нижней челюсти.

Objective

Objective. Improving the effectiveness of treatment of patients with TMJ PDS myogenic origin.

Material and methods

Material and methods. According to the purpose of the study, a group of 10 patients (8 women and 2 men aged 14 to 60 years) was formed with the coincidence of a triad of signs: pain during movements of the lower jaw, a feeling of heaviness and tension in the chewing and facial muscles, restriction of mouth opening. A set of diagnostic measures has been implemented in the following scope: clinical and radiological examination methods, TMJ MRI, ultrasound and surface EMG of the chewing muscles, digital axiography. In the treatment of patients in the study group, the method of invasive carboxytherapy was used, based on dosed injections of CO2 in the projection of trigger points of the masticatory muscles. The research examines the therapeutic value of using of CO2 in the protocol of functional rehabilitation of patients with TMJ diseases of myogenic origin.

Results

Results. The analysis of the effectiveness of treatment of patients in the study group was carried out to assess the dynamics of key parameters: the severity of the pain syndrome according to VAS, the magnitude of the IMPACT index values and the magnitude of mouth opening. The severity of pain in patients in the study group, according to average values, decreased by 86% from the initial values, an increase in the amplitude of mouth opening by 28.5%, a decrease in the IMPACT index values by 42.5% from the initial values of hypertonicity of the masticatory muscles, an increase in blood flow speed of superficial temporal artery at 52.5%.

Conclusion

Conclusion. The results of the clinical application of the carboxytherapy method in the treatment of patients with TMJ PDS allow us to recommend this method for implementation in the rehabilitation protocols of patients with TMJ PDS of myogenic origin, trismus and hypertonus of the masticatory muscles. Carboxytherapy as a method of etiopathogenetic rehabilitation contributes to the early relief of pain syndrome, normalization of the bioelectric activity of the chewing muscles and restoration the physiological kinematics of the lower jaw’s movements.
