На сегодняшний день лечение ГА у детей, ограничивается использованием в большинстве случаев наружной гормональной терапии в виде мазей либо назначением внутрикожных инъекции ГКС в амбулаторных условиях. Однако данные об эффективности данной группы препаратов при тяжелых формах течения ГА до конца не изучены. Остается открытым вопрос выбора наиболее предпочтительного лекарственного средства у пациентов одновременно с тяжелым течением ГА и тяжелой формой атопического дерматита. Еще одним из интересных вариантов лечения ГА является препарат Дупилумаб. (блокатор ИЛ-4 и ИЛ-13). Механизм действия дупилумаба при ГА остается неизвестным, предполагается его воздействие на TH 2-типа.</p><sec><title>Цель исследования</title><p>Цель исследования. Провести сравнительную характеристику эффективности: наружного ГКС (Клобетазол) и инъекционного ГКС (Бетаметазон) для лечения ГА у детей; лечения Тофацитинибом детей с гнездной алопецией и наличием или отсутствием атопического дерматита в виде сопутствующего заболевания; использования Дупилумаба для лечения детей с ГА.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследовательскую выборку включено 420 пациентов. По характеру поражения, нами рассматривалось три вида алопеции: очаговая, тотальная и универсальная. Данные пациентов взяты из регистра пациентов ГА у детей. Эффективность лечения оценивалась по шкале SALT. Наблюдение за пациентами проводилось в течение 6 месяцев.</p></sec><sec><title>Результаты</title><p>Результаты. Использование наружного ГКС (Бетаметазон), эффективнее использования наружного ГКС (Клобетазол). У пациентов с атопическим дерматитом эффективность Тофацитиниба для лечения ГА достоверно выше, чем у пациентов без атопического дерматита. Дупилумаб может являться препаратом выбора для лечения пациентов с тотальной и универсальной формой ГА.</p></sec><sec><title>Заключение</title><p>Заключение. Для оценки долгосрочной эффективности таких препаратов как Тофацитиниб и Дупилумаб необходимы дополнительные рандомизированные контролируемые клинические исследования.</p></sec></abstract><trans-abstract xml:lang="en"><p>Alopecia areata (АА) is an autoimmune inflammatory disease with an established genetic predisposition, which is based on the lesion of hair follicles, leading to persistent or temporary hair loss. To date, treatment of АA in children, is limited to the use in most cases of external hormonal therapy in the form сream or the prescription of intradermal injections of GCS in outpatient conditions. However, data on the effectiveness of this group of drugs in severe forms of AA flow are not fully studied. There is an open question of the choice of the most preferred drug in patients simultaneously with a severe flow of AA and a severe form of atopic dermatitis. Another interesting variant of treatment of AA is the drug Dupilumab. (blocker IL-4 and IL-13). The mechanism of action of dupilumab at АA remains unknown, and its effect on TH-type 2 is assumed.</p><sec><title>Purpose of the study</title><p>Purpose of the study. To conduct a comparative performance characterization of: external GCS (Clobetazole) and injectable GCS (Betamethazone) for treatment of AA in children; Tofacitinib treatment for children with alopecia and the presence or absence of atopic dermatitis in the form of a concomitant disease; use of Dupilumaba to treat children with AA.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The research sample includes 420 patients. By the nature of the lesion, we considered three types of alopecia: spotted, total and universal. Patient data is taken from the register of AA patients in children. [<xref ref-type="bibr" rid="cit13">13</xref>]. The effectiveness of the treatment was assessed on the scale SALT [<xref ref-type="bibr" rid="cit14">14</xref>]. Patients were monitored for six months.</p></sec><sec><title>Results</title><p>Results. Use of intradermal injections GKS (Betamethazone), more effective than outdoor GKS (Clobetazole). In patients with atopic dermatitis, the efficacy of Tofacitinib for AA treatment is reliably higher than in patients without atopic dermatitis. Dupilumab may be a drug of choice for treating patients with a total and universal form of AA.</p></sec><sec><title>Conclusion</title><p>Conclusion.
