Язвенный колит является одной из наиболее серьезных и малоисследованных проблем современной гастроэнтерологии. Данная нозология представляет собой диффузное воспаление слизистой оболочки кишечника, ограниченное пределами толстой кишки с обязательным вовлечением в процесс прямой кишки. Из-за прогрессирующего характера заболевания пациентам требуется пожизненная медикаментозная терапия, обследование и динамическое наблюдение. Заболевание отличается длительным упорным течением, внекишечными проявлениями и тяжелыми осложнениями, а также высоким уровнем инвалидизации пациентов. Внекишечные проявления нередко встречаются у больных язвенным колитом и представляют собой сложную проблему, которая требует пристального внимания специалистов и назначения современного патогенетически-обоснованного лечения, в том числе генно-инженерными биологическими препаратами. Данное клиническое наблюдение отражает важность ранней диагностики язвенного колита с внекишечными проявлениями – первичным склерозирующим холангитом и артропатией, а также эффективность лечения устекинумабом.

Ulcerative colitis (UC) is one of the most serious and poor-studied problems of modern gastroenterology. This nosology is a diffuse inflammation of the intestinal mucosa, limited to the colon with a predominant lesion of the rectum and sigmoid colon. Due to the progressive nature of the disease, patients require lifelong drug therapy and monitoring. Extra-intestinal manifestations are often found in patients with UC and represent a complex problem that requires the close attention of specialists and the appointment of modern pathogenetically-based treatment, including genetically engineered biological drugs. This clinical observation reflects the importance of early diagnosis of UC with extra-intestinal manifestations – primary sclerosing cholangitis and arthropathy, as well as the effectiveness of treatment with ustekinumab.
