Основное внимание авторы уделяют сложностям и противоречиям в определении возрастных рамок к проведению маммографического скрининга РМЖ, существующим в научной литературе. Цель. Определить соотношение показателей заболеваемости РМЖ в разных возрастных группах. Материалы и методы. Проанализированы данные московского канцер-регистра за 2013-2015 годы. Сравнивалось количество пациенток с впервые выявленным РМЖ в возрасте 40-74 лет и в возрасте 75 лет и старше. Для оценки значимости отличий использован U-критерий Манна-Уитни. Результаты. Показатель выявляемости РМЖ в группе женщин в возрасте 75 лет и старше был в 1,65 раза выше, чем в возрастной группе 40-49 лет. Анализ U-критерия показал, что между возрастными группами от 40 до 74 лет и возрастными группами старше 75 лет отсутствуют статистически значимые отличия в количестве случаев впервые выявленного РМЖ (р &gt; 0,05). Статистически значимые отличия (р &gt; 0,05) между количеством новых случаев РМЖ в разных возрастных группах отсутствуют вне зависимости от стадии РМЖ. Чем более поздняя стадия РМЖ диагностирована, тем слабее статистическая связь с возрастом пациентки, в котором он был выявлен. Вывод. Проведение маммографического скрининга РМЖ у женщин в возрасте 75 лет и старше целесообразно при условии соблюдения индивидуального подхода к включению пациентки в программу скрининга.</p></abstract><trans-abstract xml:lang="en"><p>Article describes current features of early breast cancer diagnosis in older women. Authors put their attention on main problems and controversies of breast cancer (BC) mammographic screening in older women. Aim was to define relationship between BC incidence in different age groups. Material and methods. Authors analyzed data from Moscow Cancer Registry in 2013-2015. BC incidence in patients 40-74 old and older than 75 years was compared. We used Mann-Whitney U-test for estimation of differences between these groups. Results. BC incidence in women 75 years old or older was in 1.65 times higher than in women 40-74 years old. U-test analysis revealed that there were no differences in amount of firstly diagnosed BC between age groups 40-74 old and older than 75 years (p &gt; 0,05). Also amounts of new BC cases in different age groups were no significant differences (p &gt; 0,05). Age of BC diagnosis dependence on patient age was weaker in patients with late stages of BC. Conclusion. BC mammographic screening should be performed in women 75 years or older.
