Малоинвазивный доступ, лучшая переносимость операции пациентами и снижение риска развития послеоперационных осложнений стали неоспоримым преимуществом в сравнении с открытыми хирургическими методиками. К сожалению, подавляющее большинство аортальных стенозов имеют общий этиологический фактор с ишемической болезнью сердца, что обусловливает необходимость инвазивного лечения поражений коронарного русла. Существует ряд сложностей, связанных с катетеризацией устья артерий и обеспечением стабильного положения инструментов после установки эндопротезов. Не всегда возможно проведение ангиопластики из «диагностического» радиального доступа, и возможно расширение на бедренный доступ с целью обеспечения более безопасного и стабильного положения инструментов во время манипуляций.</p></abstract><trans-abstract xml:lang="en"><p>Transcatheter aortic valve implantation is the main treatment for severe aortic stenosis in patients with high surgical risks. There are undeniable advantages such as minimally invasive access, better portability in patients and decreased risk of postoperative complications compare to open surgery. Unfortunately, most aortic stenoses have the same etiology with coronary artery diseases and it necessitates invasive treatment. Cardiac artery catheterization and providing of stable instrument position are complications in post TAVI patients.
