В статье проанализированы данные литературы о органосохраняющих и онкопластических резекциях молочной железы при раке. Органосохраняющие операции выполняли у пациенток со стадией I–IIА, сравнивая результаты лечения с пациентками после радикальных мастэктомий. Онкопластические резекции применяют для достижения хороших эстетических результатов при операбельном и местнораспространенном раке молочной железы, а также при такой эстетически неблагоприятной локализации опухолевого узла в молочной железе, которая заведомо приведет к неудовлетворительным косметическим результатам при классической органосохраняющей операции. В последние годы появился термин «экстремальная онкопластическая резекция», при которой расширились показания к выполнению органосохраняющих операций, а именно – при мультицентричности, мультифокальности и размере опухолевого узла более 50 мм стало возможным выполнить онкопластические резекции. Данные операции повышают качество жизни у пациенток РМЖ и являются методом реабилитации.

The article analyzes the literature data on organ-preserving and oncoplastic breast resections in cancer. Organ-preserving operations were performed in patients with stage I–IIA, comparing the results of treatment with patients after radical mastectomies. Oncoplastic resections are used to achieve good aesthetic results with operable and locally advanced breast cancer, as well as with such an aesthetically unfavorable localization of the tumor node in the mammary gland, which will obviously lead to unsatisfactory cosmetic results with classical organ-preserving surgery. In recent years, the term extreme oncoplastic resection has appeared, in which the indications for performing organ-preserving operations have expanded, namely, with multicentricity, multifocality and a tumor node size of more than 50 mm, it has become possible to perform oncoplastic resections. These operations improve the quality of life in breast cancer patients and are a method of rehabilitation.
