Поражения внесуставных мягких тканей занимают ведущее место в структуре заболеваний опорнодвигательной системы в первичном звене здравоохранения. Вместе с тем этой патологии уделяется недостаточно внимания, что приводит к снижению качества оказания помощи пациентам с внесуставной патологией.</p></sec><sec><title>Цель обзора</title><p>Цель обзора. Повышение информированности специалистов общей практики о клинических формах заболеваний внесуставных мягких тканей (ЗВМТ), современных методах диагностики и лечения</p></sec><sec><title>Основные положения</title><p>Основные положения. В статье представлена характеристика основных форм периартикулярных поражений – периартритов, в том числе плече-лопаточного периартрита, тендинитов, теносиновитов, капсулитов, бурситов. Дан анализ ведущих механизмов развития этих заболеваний, описаны клиническая картина, диагностические признаки отдельных форм. Отмечена роль современных методов визуализации периартикулярных поражений с помощью УЗИ иМРТ.Представлены основные подходы к лекарственной терапии ЗВМТ, в томчисле применение медленно действующих симптом-модифицирующих препаратов (SYSADOA). Приведены собственные данные, демонстрирующие нарушение метаболизма основного вещества соединительной ткани при ЗВМТ, а также эффективность препарата Алфлутоп в лечении околосуставных поражений «механического» характера.</p></sec><sec><title>Заключение</title><p>Заключение. Заболевания околосуставных мягких тканей как первичная патология являются следствием микротравматизации и могут быть отнесены к «механическим» формам поражения. В объективной диагностике тяжести поражения и его характера важную роль играет УЗИ с допплерографией. При ЗОМТ в крови пациентов могут быть обнаружены биомаркеры повреждения соединительной ткани. Для повышения эффективности лечения целесообразно использовать препараты из группы SYSADOA.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Lesions of extra-articular soft tissues occupy a leading place in the structure of diseases of the musculoskeletal system in primary health care. At the same time, insuffcient attention is paid to this pathology, which leads to a decrease in the quality of care for patients with extra-articular pathology.</p></sec><sec><title>Objective</title><p>Objective. Increasing the awareness of general practitioners about the clinical forms of extra-articular soft tissue diseases, current approaches to diagnosis and treatment.</p></sec><sec><title>Basic provisions</title><p>Basic provisions. The article presents the characteristics of the main forms of periarticular lesions: periarthritis, incl. painful shoulder, tendonitis, tenosynovitis, capsulitis, bursitis. The leading mechanisms of periarticular disorders are presented, the clinical picture and diagnostic signs of particular forms are described. The role of modern methods of visualization of periarticular lesions using ultrasound and MRI is noted. The main approaches to the drug therapy of periarticular disorders are presented, including use of slow-acting symptom-modifying drugs (SYSADOA). The authors present their own data demonstrating the connective tissue metabolism disturbance in periarticular disorders as well as the effectiveness of the drug Alﬂutop in the treatment of periarticular lesions of a ‘mechanical’ nature.</p></sec><sec><title>Conclusions</title><p>Conclusions. Primary forms of periarticular disorders are a consequence of microtraumatization and can be attributed to ‘mechanical’ types of damage. The ultrasound is pivotal in the diagnosis and evaluation the severity of the lesion and its origin. Biomarkers of connective tissue damage can be detected in the blood of patients with periarticular disorders.
