Изучение серотипового, клонального состава и чувствительности к антибактериальным препаратам пневмококков у детей школьного возраста.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В 2012–2018 годах обследовано 498 школьников в возрасте от 6 до 17 лет. У детей были взяты мазки из ротоглотки, проведено бактериологическое исследование с последующим генотипированием чистых культур и определением их чувствительности к антибиотикам.</p></sec><sec><title>Результаты</title><p>Результаты. Пневмококк был выявлен у 10,6% детей. Исследуемые штаммы относились к семи серотипам и семи серогруппам. Наиболее часто встречались S. pneumoniae 6-й серогруппы и серотипа 19F (по 15,1%), 9-й серогруппы (13,2%). С меньшей частотой встречались пневмококки 33FA/37 и 3 (9,4 и 5,7%), представители серогрупп 15 и 18 (7,6 и 5,7%), серотипа 10А (3,8%). Выделенные культуры принадлежали к 20 сиквенс-типам, относящимся к 14 клональным комплексам (СС); преобладающими являлись CC156, CC447 и CC320. У 1,9% штаммов выявлена резистентность к пенициллину, у 13,2% – к макролидам, клиндамицину и тетрациклину. Устойчивые к антибиотикам пневмококки относились к глобальным мультирезистентным клональным комплексам 320, 315 и 156.</p></sec><sec><title>Выводы</title><p>Выводы. Распространенность S. pneumoniae среди детей школьного возраста невысока. Популяция пневмококков характеризуется серотиповым и клональным разнообразием, в том числе присутствием «инвазивных» серотипов и генотипов. Большинство штаммов сохраняют чувствительность к антибактериальным препаратам.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose. To investigate the serotype distribution, clonal structure and antimicrobial resistance of pneumococci isolated from schoolchildren.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. During the period from 2012 to 2018 we examined 498 healthy school children aged 6 to 17 years. Oropharyngeal swab was taken from each child for culture, after that all S. pneumoniae strains were genotyped for serotype and ST-type deduction (PCR and sequencing, respectively). Antimicrobial resistance was also determined.</p></sec><sec><title>Results</title><p>Results. Pneumococcal culture was positive in 10.6 % of children. S. pneumoniae isolates belonged to seven serogroups and seven serotypes. Serogroup 6 and serotype 19F strains (15.1% each), and serogroup 9 strains (13.2%) were the most prevalent. S. pneumoniae33FA/37 and 3 (9.4 and 5.7%), serogroups 15 and 18 (7.6 and 5.7%), and 10A serotype (3.8%) were determined at a lower frequency. 20 detected ST-types belonged to 14 clonal complexes (CCs); CC156, CC447, and CC320 were predominant. 1.9% of isolates were penicillin-resistant; 13.2% – macrolide-, clindamycin-, and tetracycline-resistant. S. pneumoniae antibiotic resistant strains belonged to multidrug-resistant CCs 320, 315, and 156.</p></sec><sec><title>Conclusion</title><p>Conclusion. S. pneumoniae prevalence in school children is not high. Pneumococcal population is characterized by serotype and clonal diversity including ‘invasive’ serotypes and genotypes.
