Врожденный буллезный эпидермолиз (ВБЭ) - это группа редких генетических заболеваний различной тяжести, которые характеризуются поражением эпидермиса с образованием пузырей на коже и (или) слизистых оболочек при незначительной травме. При этом клиническое течение заболевания в ряде случаев отягощается развитием недостаточности питания, что еще дополнительно ухудшает качество жизни пациентов.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценить физическое развитие и лабораторные показатели нутритивного статуса у детей с простой и дистрофической формами ВБЭ.</p></sec><sec><title>Методы</title><p>Методы. В исследование включали детей с простой и дистрофической формами ВБЭ в возрасте от 8 мес до 18 лет, проходивших лечение в ФГАУ «НМИЦ здоровья детей» Минздрава России с января 2018 по январь 2019 года. У всех пациентов оценивались антропометрические показатели, а также проводился биохимический анализ крови на кальций общий, фосфор, альбумин, магний. Данные представлены с использованием медианы (Ме) и квартилей 25 и 75 % [Q25; Q75], а также в виде среднего (М) и 95 %-ного доверительного интервала (95 % ДИ) для переменных с нормальным распределением.</p></sec><sec><title>Результаты</title><p>Результаты. Всего в исследование было включено 87 детей, из них 70 с дистрофической и 17 с простой формой ВБЭ. Различий по полу и возрасту между группами не было. По результатам исследования все антропометрические показатели (WAZ, HAZ и BAZ) у детей с дистрофической формой ВБЭ были статистически значимо ниже (p &lt; 0,001), чем у детей с простой формой ВБЭ. Хотя при обеих формах ВБЭ усредненные показатели WAZ, HAZ и BAZ оказались в пределах нормальных значений, у детей с дистрофической формой снижение HAZ менее -2 отмечалось у 22 детей (31 %), среднетяжелая белково-энергетическая недостаточность (-3 &lt; BAZ &lt; -2) была выявлена у 29 пациентов (41 %), при этом у 20 из них (29 %) была диагностирована тяжелая белково-энергетическая недостаточность (BAZ &lt; -3). Биохимические показатели оказались в пределах референтных значений, однако уровни альбумина и кальция общего были статистически значимо снижены у детей с дистрофической формой ВБЭ по сравнению с простой формой (p &lt; 0,001 и p = 0,005 соответственно), гипоальбуминемия наблюдалась в 64 % случаев, а гипокальциемия - в 9 % при дистрофической форме ВБЭ.</p></sec><sec><title>Заключение</title><p>Заключение. Все антропометрические показатели у детей с дистрофической формой ВБЭ ниже, чем при простой форме. Исследование уровня альбумина и кальция общего в крови при дистрофической форме ВБЭ может выявить доклинические формы недостаточности питания.</p></sec></abstract><trans-abstract xml:lang="en"><p>Congenital epidermolysis bullosa (CEB) is a group of rare genetic diseases of varying severity, which are characterized with the formation of blisters on the skin and/or mucous membranes with minor trauma. In this case, the clinical course of the disease in some cases is aggravated by the development of malnutrition, which further worsens the quality of life of patients.</p><sec><title>Aim of the study</title><p>Aim of the study. To assess the physical development and laboratory findings of nutritional status in children with simple (SCEB) and dystrophic forms of CEB (DCEB).</p></sec><sec><title>Methods and materials</title><p>Methods and materials. The study included children with SCEB and DCEB at the age of 8 months to 18 years old, treated at the National Medical Research Centre of Children's Health (Moscow, Russia) from January 2018 to January 2019. Anthropometric parameters were assessed in all patients, and a biochemical blood test was performed for total calcium, phosphorus, albumin, and magnesium. Data are presented using the median (Me) and quartiles of 25 % and 75 % [Q25; Q75], as well as the mean (M) and 95 % confidence interval (95 % CI) for variables with a normal distribution.</p></sec><sec><title>Results</title><p>Results. A total of 87 children were included in the study, 70 with DCEB and 17 with SCEB. There were no differences in sex and age between the groups. According to the results of the study, all anthropometric parameters (WAZ, HAZ and BAZ) in children with DCEB were statistically significantly lower (p &lt; 0.001) than in children with SCEB. Although in both forms of CEB the averaged values of WAZ, HAZ and BAZ turned out to be within the normal range, in children with DCEB, a decrease in HAZ less than -2 was noted in 22 children (31%). Moderate protein-energy malnutrition (-3 &lt; BAZ &lt; -2) was detected in 29 patients (41 %), while 20 of them (29%) were diagnosed with severe protein-energy malnutrition (BAZ &lt; -3). Biochemical parameters were within the reference values, however, the levels of albumin and total calcium were statistically significantly reduced in children with DCEB compared with SCEB children (p &lt; 0.001 and p = 0.005, respectively). Hypoalbuminemia was observed in 64 %, and hypocalcemia in 9 % of DCEB children.</p></sec><sec><title>Conclusion</title><p>Conclusion. All anthropometric values in children with DCEB were lower than in children with SCEB.
