Многие специалисты откладывают его назначение до ситуации, когда все другие варианты системной терапии исчерпаны, физическое состояние пациента близко к неудовлетворительному, а метастатический процесс достигает максимального объема. Позиция онкологов, придерживающихся такой позиции, основана на небольшой частоте объективных ответов и невысоких показателях выживаемости без признаков прогрессирования процесса, при наличии свидетельств о наличии большей эффективности повторного назначения уже использованных в лечении данного пациента химио‑ и таргетных препаратов. С другой стороны, анализ мировой литературы, посвященной регорафенибу и вариантам III линии терапии метастатического колоректального рака, показывает неоднозначность вышеуказанного подхода и потенциал препарата с точки зрения улучшения перспектив пациента за счет преодоления резистентности опухолевой массы к уже использованным лекарственным препаратам и возможности предоставить пациенту так называемые химиотерапевтические каникулы.</p></abstract><trans-abstract xml:lang="en"><p>The use of regorafenib in the treatment of patients with metastatic colorectal cancer divides Russian and foreign oncologists into two camps. Many specialists postpone its appointment until a situation where all other options for systemic therapy have been exhausted, and the physical condition is close to unsatisfactory, and the metastatic process reaches its maximum volume. The position of oncologists adhering to such positions is based on a small number of objective answers and low survival rates without signs of progression of the process, with evidence of a more effective re-prescription of chemotherapy and targeted drugs already used in the treatment of this patient. On the other hand, an analysis of the world literature on regorafenib and options for the third line of therapy for metastatic colorectal cancer shows the ambiguity of the above approach and the potential of the drug in terms of improving the patient’s prospects by overcoming the resistance of the tumor mass to already used drugs and the ability to provide the patient with the so-called chemotherapeutic holiday.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>метастатический колоректальный рак</kwd><kwd>третья линия терапии</kwd><kwd>регорафениб</kwd></kwd-group><kwd-group xml:lang="en"><kwd>metastatic colorectal cancer</kwd><kwd>third line therapy</kwd><kwd>regorafenib</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Van Cutsem E, Cervantes A. et al.
