Клиническая картина, идентичная таковой при синдроме раздраженного кишечника, может ассоциироваться с воспалительными заболеваниями кишечника, колоректальным раком, целиакией, пищевой аллергией, лактазодефицитной энтеропатией, нецелиакийной чувствительностью, к глютену, и другими заболеваниями. В связи с этим синдром раздраженного кишечника является диагнозом исключения и всегда требует проведения сложной дифференциальной диагностики для установления истинной причины заболевания, проведения адекватной терапии и достижения стойкой ремиссии. Целью обзора явилось получение научного знания о СРК-подобных синдромах и выработка оптимальной тактики ведения данных больных. В обзоре продемонстрировано, что клинические проявления СРК во многом носят неспецифический характер из-за частого перехода одной формы заболевания в другую, что создает определенные трудности в постановке диагноза. Своевременная диагностика СРК-подобных синдромов, нередко наслаивающихся на уже имеющуюся патологию СРК, значительно улучшает результаты лечения пациентов с кишечной симптоматикой и обеспечивает длительное сохранение ремиссии заболевания.</p></abstract><trans-abstract xml:lang="en"><p>IBS-like syndromes are one of the most important problems in gastroenterology. The clinical picture is identical to that of irritable bowel syndrome can be associated with inflammatory bowel disease, colorectal cancer, celiac disease, food allergy, lactase deficiency enteropathy, non-celiac sensitivity, gluten and other diseases. In this regard, irritable bowel syndrome is a diagnosis of exclusion and always requires a rather complex differential diagnosis to establish the true cause of the disease, conduct adequate therapy and achieve a stable remission. The purpose of the review was to obtain scientific knowledge about IBS-like syndromes and to develop optimal management tactics for these patients. The review demonstrates that the clinical manifestations of IBS are largely nonspecific due to the frequent transition of one form of the disease to another, which creates certain difficulties in making a diagnosis.
