Подходы к лечению рака предстательной железы (РПЖ) значительно изменились за последнее десятилетие. В настоящее время РПЖ является одним из наиболее распространенных онкологических заболеваний и второй по частоте встречаемости причиной смерти мужчин. Частота этого заболевания увеличивается с возрастом, в связи с чем врачи в повседневной работе все чаще сталкиваются с проблемой лечения РПЖ именно у пациентов старшего возраста. Как показывают данные многочисленных исследований, при тщательно подобранной терапии РПЖ выживаемость пожилых пациентов и пациентов боле молодого возраста аналогична. Тем не менее у пациентов старшего возраста общее состояние здоровья и сопутствующие заболевания могут негативно влиять на переносимость как гормональной, так и химиотерапии, а также усугублять токсическое воздействие последней. Более того, гетерогенность этой популяции пациентов требует мультидисциплинарного подхода для выбора тактики ведения (активное наблюдение или поддерживающая терапия) и определения пользы консервативного и (или) оперативного лечения при РПЖ.

The approaches to the treatment of prostate cancer (PCa) has changed considerably over the last decade. Currently, prostate cancer is one of the most common malignancy and the second leading cause of death among men. The incidence of this disease increases with age, that is why doctors are increasingly facing problems with prostate cancer treatment in older patients. In numerous studies it was shown that carefully selected prostate cancer treatment provides similar survival results of elderly and younger patients. However, in older patients overall health and related diseases can adversely affect the efficacy and safety of different treatment methods. Furthermore, the heterogeneity of the patients population requires a multidisciplinary approach to select proper treatment modality (active surveillance or maintenance therapy) and to determine the use of the conservative and (or) surgical treatment of prostate cancer.
