Детекция минимальной остаточной болезни (МОБ) при хроническом лимфолейкозе (ХЛЛ) методом многоцветной проточной цитометрии нашла широкое применение в клинических исследованиях для оценки эффективности лечения. Метод совершенствуется в направлении поиска наиболее чувствительных и специфичных маркеров для применения в панели для 6–8 цветных цитометров. По литературным данным, ROR1 демонстрирует высокую экспрессию на ХЛЛ-клетках и отсутствие экспрессии на зрелых лимфоцитах, что отличает его от других маркеров, применяющихся для выявления МОБ при ХЛЛ.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: определение информативности маркера ROR1 в четырехцветной панели для оценки минимальной остаточной болезни при ХЛЛ методом проточной цитометрии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проанализировано 64 образца костного мозга 37 пациентов с верифицированным диагнозом ХЛЛ после 3-го и 6-го курса терапии по схеме «бендамустин + ритуксимаб» (BR) – 15 МОБ-отрицательных и 49 МОБ-положительных. Количественное определение МОБ проводилось стандартизованным методом четырехцветной проточной цитомерии по рекомендациям ERIC (Европейская инициатива по изучению ХЛЛ) с включением в диагностическую панель маркера ROR1. Выполнен дискриминационный анализ дифференцирующих свойств диагностических маркеров с помощью программы Statistica 10.</p></sec><sec><title>Результаты</title><p>Результаты. ROR1 продемонстрировал высокие дифференцирующие свойства в отношении ХЛЛ-клеток и зрелых лимфоцитов. Во всех проанализированных образцах отмечались яркая мономорфная экспрессия ROR1 на ХЛЛ-клетках и В-клеточных предшественниках и отсутствие его экспрессии на зрелых лимфоцитах.</p></sec><sec><title>Выводы</title><p>Выводы. ROR1 является высокоспецифичным и чувствительным маркером для выявления ХЛЛ-клеток среди зрелых лимфоцитов. Высокая экспрессия ROR1 на нормальных В-клеточных предшественниках требует применения ROR1 в сочетании с маркером, дифференцирующим ХЛЛ-клетки от клеток-предшественников (CD81).</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background and Aims</title><p>Background and Aims. The detection of minimal residual disease (MRD) of chronic lymphocytic leukemia (CLL) using multicolor flow cytometry has been widely used in clinical studies to evaluate the effectiveness of treatment. The method is being improved by searching for the most sensitive and specific markers for use in panels for 6–8 color cytometers. According to published data, ROR1 shows high expression on CLL cells, and lack of expression on mature lymphocytes, which distinguishes it from other markers used to detect MRD in CLL.</p></sec><sec><title>Aim</title><p>Aim: to determine a significance of ROR1 for detection of MRD CLL by flow cytometry in a 4-color panel.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. We analyzed 64 bone marrow samples of 37 patients with a verified diagnosis of CLL after the 3rd and 6th cycles of therapy according to bendamustine and rituximab (BR) regimen – 15 MRD-negative and 49 MRD-positive. Quantitative determination of MRD was carried out by the standardized method of 4-color flow cytometry according to the recommendations of ERIC (European research initiative on CLL) with the inclusion of ROR1 in the diagnostic panel. A discriminatory analysis of the differentiating properties of diagnostic markers was performed using Statistica 10.</p></sec><sec><title>Results</title><p>Results. ROR1 has demonstrated high differentiating properties on CLL cells and mature lymphocytes. All the analyzed samples showed a bright monomorphic expression of ROR1 on CLL cells and B-cell precursors, and the absence of its expression on mature lymphocytes.</p></sec><sec><title>Conclusion</title><p>Conclusion. ROR1 is a highly specific and sensitive marker for the detection of CLL cells among mature lymphocytes.
