Оценить глобальную деформацию и скорость глобальной деформации левого предсердия (ЛП) в зависимости от степени митральной регургитации (МР) у больных хронической ишемической МР. Материал и методы исследования. Анализ выполнен у 118 больных ишемической МР IIIb типом по Carpantier A. Стандартная эхокардиография и технология след пятна (Speckle Tracking Imaging) для оценки деформации (Strain) и скорости деформации Strain Rate) ЛП были выполнены на ультразвуковой системе VIVID 7 Dimension (GE, Healthcare) с использованием EchoPAC. Оценены показатели геометрии митрального клапана (МК) и объемные показатели МР, объемы и фракция выброса ЛП. Результаты. Значения деформация ЛП не связаны со степенью МР. Скорость глобальной деформации ЛП зависела от степени МР. Наименьшие значения скорости глобальной деформации ЛП выявлены при 3-й степени МР. Тентинг створок МК коррелировал со скоростью глобальной деформации ЛП (r = -0,45; p = 0,0003). Максимальный объем ЛП был связан с диаметром фиброзного кольца МК (r = 0,59), коаптационным расстоянием (r = 0,53), площадью тентинга створок МК (r = 0,56), межпапиллярной дистанцией (r = 0,51). Глобальная деформация и скорость глобальной деформации ЛП не коррелировали с максимальным объемом ЛП и фракцией выброса ЛП. Выводы. Площадь тентинга створок, диаметр фиброзного кольца, глубина и длина коаптации створок МК коррелируют с максимальным объемом ЛП и скоростью глобальной деформации ЛП у больных с ишемической МР. Значения скорости глобальной деформации ЛП зависят от степени МР. Деформация ЛП не взаимосвязана со степенью МР.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To evaluate left atrial global strain and strain rate and their correlation with mitral regurgitation severity in patients with chronic ischemic mitral regurgitation. Materials and methods. The analysis was performed in 118 patients with type Ilb ischemic mitral regurgitation according to Carpentier classification. Standard echocardiography and speckle tracking imaging were performed in order to assess left atrial global strain and strain rate. VIVID 7 Dimension (GE, Healthcare) ultrasound system and EchoPAC were used. Mitral valve (MV) geometry indices, mitral valve volumetric indices, left atrial volumes and left atrial ejection fraction were measured. Results. Left atrial strain values did not depend on mitral regurgitation severity. Left atrial global strain rate depended on mitral regur-gitation severity. The lowest values of left atrial global strain rate were presented in patients with grade 3 mitral regurgitation. Tenting of mitral leaflets correlated with left atrial global strain rate (r=-0,45; p=0,0003). Maximal left atrial volume correlated with the diameter of mitral annulus (r=0,56), coaptation depth (r=0,53), MV tenting area (r=0,56) and interpapillary muscle distance (r=0,51). Left atrial global strain and strain rate did not correlate with left atrial maximal volume and left atrial ejection fraction. Conclusions. MV tenting area, MV annular diameter, coaptation depth and coaptation heights correlate with left atrial maximal volume and global strain rate in patients with ischemic mitral regurgitation. Left atrial global strain rate depends on MR grade.
