Приблизительно у 5–10 % пациентов с серонегативными спондилоартритами в конечном итоге развивается ВЗК, причем болезнь Крона более распространена, чем язвенный колит. Для диагностики воспалительного заболевания кишечника, как правило, используется колоноскопия, однако эта процедура является трудоемкой и инвазивной. При воспалении в кишечнике повышается уровень фекального кальпротектина и его измерение может стать удобным способом диагностики.</p><sec><title>Цель исследования</title><p>Цель исследования: оценить частоту встречаемости воспалительных заболеваний кишечника у пациентов с АС в реальной клинической практике.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В ГБУЗ «МКНЦ имени А. С. Логинова» в 2018–2019 годах было обследовано 40 пациентов с АС, соответствующих модифицированным Нью-Йоркским критериям 1984 года. Среди них 26 (65 %) мужчин, 14 (35 %) женщин, средний возраст пациентов — 41,2 ± 10,5 года, средняя продолжительность заболевания 13,0 ± 8,8 года. Всем больным проводились исследования: клинический анализ крови, СОЭ, СРБ, эзофагогастродуоденоскопия, колоноскопия и количественный анализ уровня ФК с использованием метода латеральной иммунохроматографии с помощью экспресс-анализатора BUHLMANN Quantum Blue. Диапазон измерения: 100–1 800 мкг/г кала.</p></sec><sec><title>Результаты</title><p>Результаты: у 31 пациента отмечалась высокая активность основного заболевания, среднее значение BASDAI составило 5,2 ± 1,7, ASDAS СРБ — 3,8 ± 1,1. У 35 пациентов (87,5 %) уровень ФК составлял более 100 мкг/г (12 пациентов [30,0 %] — более 1 800 мкг/г, 23 пациента (57,5 %) — от 101 до 1 799 мкг/г), 5 пациентов (12,5 %) имели уровень ФК менее 100 мкг/г. В большинстве случаев у 35 пациентов с высокими значениями ФК (более 100 мкг/г) отмечался высокий уровень СРБ (среднее значение 28,4 мг/л) и СОЭ (среднее значение 36,3 мм/ч). ВЗК были диагностированы у 9 пациентов (22,5 %), из которых у 5 пациентов (12,5 %) — болезнь Крона и у 4 пациентов (10,0 %) — язвенный колит, в остальных случаях (77,5 %) патологии кишечника не выявлено.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные данные свидетельствуют о высокой частоте развития ВЗК у пациентов с АС. Показано, что у пациентов с уровнем ФК более 100 мкг/г отмечалась высокая активность основного заболевания (АС). В большинстве случаев воспалительные заболевания кишечника были диагностированы у пациентов АС с уровнем ФК более 1 800 мкг/г.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Аnkylosing spondylitis (AS) is closely associated with inflammatory bowel disease (IBD). 5–10 % of patients with SpA eventually develop inflammatory bowel disease, with Crohn's disease being more common than ulcerative colitis. Colonoscopy is usually used to diagnose inflammatory bowel disease, but this procedure is invasive. FC is clinically used to detect IBD and correlates well with clinical, endoscopic, and histological indicators of disease activity in IBD.</p></sec><sec><title>The aim</title><p>The aim. To evaluate the incidence of inflammatory bowel disease in patients with ankylosing spondylitis.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. In the analysis were included 40 patients with ankylosing spondylitis, among them 26 (65.0 %) men, and 14 (35.0 %) women, the average age of patients was 41.2 ± 10.5, the duration of the disease was on average 13.0 ± 8.8 years. All patients were examined with ESR, CRP, esophagogastroduodenoscopy, colonoscopy and quantitative analysis of the fecal calprotectin levels using the method of lateral immunochromatography with the BUHLMANN Quantum Blue rapid test. Standart range: 100–1,800 µg/g.</p></sec><sec><title>Results</title><p>Results. All patients had a high disease activity, the average BASDAI was 5.2 ± 1.7, the average ASDAS CRP 3.8 ± 1.1. 35 (87.5 %) patients had calprotectin level more than 100 µg/g, the remaining 5 (12.5 %) patients less than 100 µg/g. 12 (30.0 %) patients had the calprotectin level more than 1,800 µg/g, 23 (57.5 %) from 101 to 1800 µg/g. All patients with FC levels more than 100 µg/g showed an increase CRP level (mean 28.4 mg/l) and ESR (mean 36.3 mm/h). IBD were diagnosed in 9 (22.5 %) cases: 5 (12.5 %) patients with Crohn's disease and 4 (10 %) patients with ulcerative colitis, in the remaining (77.5 %) cases there was no intestinal pathology.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results showed high frequency of IBD in patients with AS. Patients with high fecal calprotectin levels (more than 100 μg/g) had high disease activity (AS).
