На текущий момент консенсус относительно клинически обоснованной границы верхнего значения ТТГ для здоровых людей в мировом сообществе отсутствует и, по сути, текущий РИ объединяет два диапазона: зону практически «полного благополучия» (0,4–2,5 мкМЕ/мл) и зону латентной патологии и (или) «высоконормальные» значения (2,5–4,0 [5,0] мкМЕ/мл). Учитывая тот факт, что каждая лаборатория должна сама определить РИ для измеряемых аналитов, исходя из особенностей обследуемого контингента, характеристик используемых аналитических систем, достижения необходимой диагностической эффективности методики, собственных практических и экономических возможностей, целью данной работы явилось ретроспективное определение РИ ТТГ для российской популяции на основании анализа 400 394 заказов, включающих ТТГ, произведенных в течение года наблюдений. Последовательное усложнение критериев исключения и алгоритмов расчета позволило условно выделить семь анализируемых групп и этапов определения РИ, по итогам каждого из которых были определены промежуточные и итоговые результаты исследования.</p></abstract><trans-abstract xml:lang="en"><p>Among all hormonal studies, thyroid-stimulating hormone (TSH) remains the most often deviating from reference intervals (RI), despite the fact that the analytical characteristics of methods for assessing its concentration are improved from year to year, and the functional sensitivity of each subsequent generation is improved compared to the previous one. Currently, there is no consensus in the world community on the clinically justified limit of the upper TSH value for healthy people and, in fact, the current RI combines two ranges: the zone of almost ‘complete well-being’ (0.4–2.5 μMU/ml) and the one of latent pathology and/or ‘highly normal’ values (2.5–4.0 [5.0] μIU/ml). Considering the fact that each laboratory should determine the RI for measured analytes on the basis of the characteristics of the examined population, the characteristics of the used analytical systems, the achievement of the necessary diagnostic efficiency of the technique, its own practical and economic possibilities, the aim of this work was to retrospectively determine the TSH RI for the Russian population based on the analysis of 400 394 orders, including TSH, made during one year of observation.
