Сравнить годовые результаты консервативной и инвазивной тактик лечения инфаркта миокарда (ИМ) у пациентов с различной выраженностью коморбидного фона.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Представленные результаты основаны на данных регистра ГБУЗ «Городская больница № 4 г. Сочи». В настоящий анализ вошли пациенты с диагнозом «инфаркт миокарда» (n = 1 176). Всем пациентам при выписке из стационара проведен анализ выраженности коморбидности с помощью индекса CCI (Charlson comorbidity index). Через год у 791 пациента удалось выяснить прогноз после выписки из стационара. Годовая летальность составила 12,6 % (n = 100).</p></sec><sec><title>Результаты</title><p>Результаты. Частота проведения коронарной ангиографии (КАГ) и чрескожного коронарного вмешательства (ЧКВ) у больных с CCI = 0 (нет или минимальная коморбидность, n = 408) составила 84,3 и 63,5 %, при значении CCI 1–2 (умеренная коморбидность, n = 438) —  68,0 и 44,8 %, при CCI ≥ 3 (выраженная коморбидность, n = 330) —  50,3 и 25,8 % соответственно. При этом проведение ЧКВ на госпитальном этапе наблюдения ассоциировалось со снижением летальности в течение года после выписки из стационара в 3,2 раза (с 18,5 до 5,8 %; р &lt; 0,0001). Наибольший позитивный эффект ЧКВ а отношении оптимизации отдаленного прогноза (1 год) был достигнут в группе пациентов с выраженной коморбидностью (CCI ≥ 3), где показатель NNT (number needed to treat —  число больных, которых необходимо пролечить) составил 7. Относительный риск годовой летальности у пациентов с выраженной коморбидностью в сравнении с минимальной в группе ЧКВ составил 6,75, в группе консервативного лечения —  4,63.</p></sec><sec><title>Заключение</title><p>Заключение. Результаты настоящего исследования показали, что ЧКВ при ИМ чаще выполняется более молодым, менее коморбидным пациентам с более низким риском по шкале GRACE. При этом годовая выживаемость больных с ИМ была значительно выше после ЧКВ в сравнении с первичной консервативной стратегией лечения независимо от выраженности коморбидности, а наибольший улучшающий отдаленный прогноз эффект ЧКВ имел место именно у пациентов с выраженной коморбидностью, определяемой по индексу CCI ≥ 3.</p></sec></abstract><trans-abstract xml:lang="en"><p>The aim of the study was to compare the 1-year results of conservative and invasive tactics of treating myocardial infarction (MI) in patients with different severity of comorbid background.</p><sec><title>Material and methods</title><p>Material and methods. The presented results are based on registry in City Hospital No. 4 of Sochi. The present analysis included patients with a diagnosis of myocardial infarction (n = 1 176). Upon discharge from the hospital, all patients underwent analysis of the severity of comorbidity using the Charlson Comorbidity Index (CCI). A year later, 791 patients managed to find out the prognosis after discharge from the hospital. 1-year mortality was 12.6 % (n = 100).</p></sec><sec><title>Results</title><p>Results. The frequency of coronary angiography (CA) and percutaneous coronary intervention (PCI) in patients with CCI = 0 (no or minimal comorbidity, n = 408) was 84.3 % and 63.5 %, with a CCI of 1–2 (moderate comorbidity), n = 438) was 68.0 % and 44.8 %, with CCI ≥ 3 (expressed comorbidity, n = 330) was 50.3 % and 25.8 %, respectively. At the same time, conducting PCI at the hospital observation stage was associated with a decrease mortality during the year after discharge from the hospital from 18.5 % to 5.8 %, p &lt; 0.0001. The greatest positive effect of PCI for optimizing the long-term prognosis of (1 year) was achieved in the group of patients with severe comorbidity (CCI ≥ 3), where the NNT (number needed to treat) was 7. The relative risk of 1-year mortality in patients with severe comorbidity compared to the minimum in the PCI group was 6.75, in the conservative treatment group was 4.63.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results of this study showed that PCI in MI is more often performed by younger, less comorbid patients with a lower risk on the GRACE scale.
